Title:FIZIČNA IN PSIHIČNA PRIPRAVA BOLNIKA NA BRONHOSKOPIJO

Abstract:V diplomskem delu sta prikazani anatomija in fiziologija pljuč, opisane pa so najpogosteje izvajane invazivne diagnostične in terapevtske metode v pulmologiji, s poudarkom na bronhoskopiji. Sodobna medicinska oprema in izurjena bronhoskopska ekipa bolniku bistveno olajšata to preiskavo, vendar je tako s psihičnega kot fizičnega vidika za bolnika še vedno zelo naporna in stresna. Najprimernejša zdravstvenovzgojna metoda v sklopu psihične priprave na bronhoskopijo je voden razgovor z bolnikom. Za zmanjšanje strahu pred preiskavo strokovnjaki priporočajo takšen pristop, pri katerem bolnik dobi informacije v avditivni in vizualni obliki. Pri fizični pripravi bolnika mora medicinska sestra upoštevati standarde zdravstvene nege. V empiričnem delu je predstavljena raziskava, ki je bila izvedena med 78-imi bolniki, ki smo jim v Bolnišnici Topolšica opravili bronhoskopijo marca in aprila 2011. Ugotoviti smo želeli, ali je psihična priprava bolnika na bronhoskopijo v tej ustanovi skladna s sodobnimi smernicami za pripravo na tovrstno preiskavo in ali so bolniki zadovoljni s pristopom oz. odnosom medicinske sestre ter z njeno komunikacijo. Ugotovili smo, da zdravstveno osebje v tej ustanovi neposredno pred preiskavo bolniku potek samo opiše in da ob tem ne uporablja nobenega pripomočka. Kljub temu si večina bolnikov dobro predstavlja, kako bo preiskava potekala in si mnogi med njimi pred tem niti ne bi želeli ogledati videoposnetka. Pri večini bolnikov se po pogovoru z medicinsko sestro zmanjša strah pred preiskavo. Zadovoljni so tudi z opremo bronhoskopske enote. Kljub lokalni anesteziji še vedno visok odstotek bolnikov bronhoskopijo doživlja kot bolečo. Če bi bolnike, ki jih pošljejo na bronhoskopijo, zdravniki izčrpneje seznanili s potekom te preiskave in če bi medicinske sestre v sklopu psihične priprave na bronhoskopijo uporabile informativni pristop ter bolnike naučile eno od tehnik za večjo sprostitev, bi bila morda preiskava posledično zanje tudi manj boleča.

Title:PHYSICAL AND MENTAL PREPARATION OF PATIEN FOR BRONCHOSCOPY
Abstract:The anatomy and psysiology of the lungs are presented in this diploma paper. The most frequently used invasive diagnostical and therapeutic methods in pulmonology are described withe emphasis on bronchoscopy. Modern medical equipment and well-trained bronchoscopic team make the procedure much easier for the patient and they greatly reduce the possibility of complications during and after the procedure. Nevertheless the procedure is still very hard and stressful for the patient. The most proper healtheducational method in the context of psychologacal preparation for bronchoscopy is a dialog with the patient. In order the reduce the anxiety before the procedure the professionals recommend the approach which gives information in the audio and visual forms. During the psysical preparation of the patient, the nurse has to perform in accordance with standards of healt care. In the empirical part of the diploma paper the survey which was carried out in a group of 78 patients who underwent bronchoscopy in Topolšica Hospital in March and April, 2011. The main goal of the survey was to establish whether the psychological preparation of the patient for bronchoscopy in that hospital is carried out in accordance with the modern methods of preparing the patient for the procedure and whether the patients are satisfied with the approach or attitude of the nurse and with her communication. It has been established that the medical staff in this hospital only describe the procedure to the patient without using any additional audio or visual equipment just before it is carried out. Nevertheless, the majority of the patients have quite a realistic idea of what the procedure will be like and what most of them would not have wanted to watch a visual presentation of the procedure even if it had been offered to them. After having spoken with the nurse, most patients experience less anxiety regarding the procedure itself. They are also satisfied with the equipment of the bronchoscopic unit. Despite being given a local anaesthesia a high percentage of the patients find bronchoscopy painful. If the patients who undergo bronchoscopy werw informed in greater detail about the procedure by the doctors and if the nurses used the informative aproach in the context of the prepatation for the procedure and tought the patients one of the techiques for a better relaxation, they would probably experience the procedure less painfull.
