Title:Tehnološke specifičnosti implatabilnih električnih spodbujevalnikov srca in kardiverter-defibrilatorjev.

Abstract:Elektronski spodbujevalniki srca in kardioverter-defibrilatorji (ICD) so postali nepogrešljivi pri zdravljenju bolnikov z motnjami ritma srca. Te vsadne majhne naprave so lahko občutljive na zunanje motnje, ki prihajajo iz zunanjega sveta. V diplomskem delu smo raziskali vire teh zunanjih dejavnikov, ki bi lahko negativno vplivali na delovanje vsadnih naprav. Najbolj nevarna sta elektromagnetno in ionizirajoče sevanje. Ugotavljamo, da na spodbujevalnike srca in kardioverter-defibrilatorje lahko vpliva sončni vihar, ki nastane zaradi aktivnosti Sonca. Takšne sončne nevihte, kjer Zemljo dosežejo električno nabiti delci, se pojavljajo s periodo vsakih 11 let. Naslednja nevihta bo leta 2012 in ne moremo natančno napovedati, kakšna bo njena moč. Vsadne naprave so občutljive tudi na ionizirajoče sevanje iz naprav za obsevanje onkoloških bolnikov. Obstaja možnost resetiranja spodbujevalnika ali ICD-naprave tudi zunaj radiacijskega snopa. Detektorji kovin na letališčih in detektorji proti krajam blaga v veleblagovnicah niso nevarni za bolnike. Velika skrb za bolnike z ICD in spodbujevalniki je slikanje z magnetno resonanco (MRI), ki je pri teh bolnikih kontraindicirano. Dvig temperature v elektrodah je lahko tudi do 20–80 °C. Februarja 2011 je bil na tržišču predstavljen prvi spodbujevalnik (Medtronic, Revo MRI SureScan), s katerim je MRI-slikanje varno. Intenzivne raziskave so usmerjene v izdelavo prihodnjih bioloških spodbujevalnikov srca, ki temeljijo na genskem inženiringu.

Title:Technological specificity in implantable electrical pacemakers and cardioverter-defibrillators.
Abstract:Electronic pacemakers and implantable cardioverter-defibrillators (ICD) have become indispensible in treating patients with cardiac arrhythmia. These small implantable devices can be sensitive to exogenous interferences from the outside world. This diploma thesis researched the sources of these external factors that may have a negative impact on the operation of implantable devices. The most dangerous are electromagnetic and ionising radiation. It was established that pacemakers and cardioverter-defibrillators may be affected by solar flares unleashed by the sun’s activity. Such solar flares, when electrically charged particles reach the Earth, appear in 11-year cycles. The next solar flare will happen in 2012 and we are unable to exactly predict its intensity. Implantable devices are also sensitive to ionising radiation from devices for radiation of oncology patients. There is the possibility of the pacemaker or the ICD to reset even if not placed within the radiation bundle. Airport metal detectors and theft detectors in stores are not dangerous for the patients. A major concern of ICD and pacemaker patients is magnetic resonance imaging (MRI), which is contraindicated for these patients. The temperature rise at the electrode can be from 20 to 80 ˚C. In February 2011, the first pacemaker (Medtronic, Revo MRI SureScan) designed for safe use of MRI was introduced to the market. Intensive research is directed towards fabricating biological pacemakers in the future basing them on genetic engineering.
