Title:Vloga medicinske sestre pri preprečevanju napredovanja kronične ledvične bolezni pri otroku in mladostniku

Abstract:Kronična ledvična bolezen (KLB) ni tipična bolezen otroške dobe, njena pogostnost je pri otrocih in mladostnikih precej manjša kot pri odraslih. Predstavlja pomemben zdravstveni problem otrok in mladostnikov. Z zdravstveno vzgojnim delom medicinska sestra (MS) in sodelujoči zdravstveni tim pomaga do upočasnitve napredovanja KLB in do podaljšanja časa prihoda na nadomestno zdravljenje. Metodologija raziskovanja: Podatke smo zbrali s pomočjo anketnega vprašalnika, ki je vseboval 21 vprašanj. 6 vprašanj je bilo zaprtega in 15 odprtega tipa. Raziskovalni vzorec je zajemal 12 članov negovalnega tima iz Centra za otroško dializo in 10 članov negovalnega tima Kliničnega oddelka za nefrologijo, Pediatrične klinike, UKC Ljubljana. 5 članov iz obeh negovalnih timov je sodelovanje zavrnilo. Namen raziskave je bil ugotoviti, ali so starši otrok oz. mladostnikov s KLB poučeni o pomenu preddializne edukacije in s katerimi težavami otrok oz. mladostnikov in njihovih staršev se medicinske sestre srečujejo v preddializni edukaciji. Rezultati raziskave: Ugotovili smo, da so po mnenju 76 % anketiranih zaposlenih starši otrok oz. mladostnikov s KLB dokaj dobro seznanjeni z namenom preddializne edukacije: preprečevanje napredovanja KLB pri otroku in mladostniku. Ugotovili smo, da 32 % anketiranih izvaja največ ZV dela na področju dietne prehrane, 32 % vnosu tekočine v telo in 26 % uživanju predpisane medikamentozne terapije. Sklep: S strani MS in zdravstvenega tima bo potrebno še veliko poučevanja otrok oz. mladostnikov s KLB in njihovih družin. Otroci in mladostniki s KLB in njihove družine morajo upoštevati vse ukrepe za upočasnitev napredovanja KLB, saj podaljšujejo čas prihoda na nadomestno zdravljenje, ob enem pa predstavljajo aktivno vključevanje v proces zdravljenja.

Title:The role of the nurse in preventing the progression of chronic kidney disease in children and adolescents
Abstract:Chronic kidney disease (CKD) is not a typical disease of childhood. Its incidence is in children and adolescents much lower than in adults but still represents an important health problem of children and adolescents. The work of health education nurse and co- medical team help to slow down progression of CKD and to extend the time to the alternate treatment. Methodology of research: Data were collected trought a survey questionaire, which contained 21 questions. The research sample consisted 12 members of the nursing team from the Centre for Children′s Dialysis, University Medical Centre Ljubljana, and 10 members of the nursing team, Department of Nephrology, Pediatric Clinic, University Medical Centre Ljubljana. Five members from this two nursing teams refused to cooperate. The aim of our study was to determine the awareness of parents of children and adolescents with CKD about the importance of pre- dialysis education and to define the problems of children or adolescents with CKD and their parents which are the subject of nurses′ pre- dialysis education. Results: According to the questionaire, 76 % of members of the nursing teams agreed that the parents of children and adolescents with CKD are fairly well acquainted with the purpose of education before to dialysis: prevention of progression of CKD in children and adolescents. We found that 32 % of enrolled members of the teams carried out of health education work in the field of diet nutrition, 32 % fluid intake, and the prescribed medical therapy in 26 %. Conclusion: From the nurses and medical teams side a lot of education of children and adolescents with CKD and their families will be needed. The children and adolescents with CKD and their families need to consider all measures to slow the progression of CKD, to delay the time of arrival on replacement therapy, and to be at the same time actively involved in the healing process.
