Title:Komunikacija medicinske sestre z dementnim pacientom

Abstract:Izhodišča: Demenca predstavlja hudo duševno motnjo in sodi med najpogostejše motnje v starosti, s tem pa prinaša veliko težav in stisk ne le obolelemu, pač pa tudi njegovim svojcem. Spremljanje in skrb za ljudi z demenco je lahko zelo stresno in izredno težko. Še posebej težava pri komunikaciji zahteva od medicinskih sester veliko strokovnega znanja, potrpljenja in vztrajnosti. Namen raziskave je bilo ugotoviti, s kakšnimi težavami se srečuje zdravstveno osebje med pogovorom z dementnim pacientom, katere okoliščine vplivajo na pogovor, in navesti najpogostejše ovire, ki onemogočajo uspešno sporazumevanje. Metode: Za raziskavo je bila uporabljena deskriptivna metoda dela, kot raziskovalni instrument smo uporabili anketni vprašalnik kombiniranega tipa. Kvantitativne podatke smo statistično obdelali s pomočjo Microsoft Excel ter rezultate ponazorili v obliki tortnih grafikonov in naloženega grafičnega grafikona. Rezultati: V raziskavi smo ugotovili, da se skoraj vsi zaposleni srečujejo s težavami pri komunikaciji z dementnimi pacienti. Te težave so, da pacienti ne najdejo pravih besed, da zaposlenih ne želijo poslušati in da pacienti govorijo ponavljajoče besede. Iz tega je razvidno, da se težave pojavljajo kontinuirano in je potrebno biti pri delu dosleden in strokoven. Pri komuniciranju zaposleni pacienta pokličejo po imenu, uporabljajo enostavne stavke in govorijo počasi. Pri nebesedni komunikaciji pa zaposleni največkrat vzpostavijo očesni kontakt, uporabijo mimiko obraza in govorico telesa. Prav tako smo ugotovili, da se zaposleni kljub strokovnemu znanju, izkušnjam in motivaciji pri delu z dementnimi pacienti srečujejo z obremenjenostjo ob koncu delovnega dne ter da delovna organizacija nudi premalo strokovnega izobraževanja s področja psihiatrične gerontologije. Razprava in zaključki: Uspešna komunikacija medicinske sestre z dementnimi pacienti je ključnega pomena za nadaljnji potek bolezni. Za uspešnejše delo s pacienti z demenco in njihovimi svojci mora medicinska sestra poleg simptomov poznati tudi stadije demence s spremljajočimi simptomi ter se stalno izobraževati o bolezni. To je podlaga za individualni pristop v skrbi za pacienta z demenco.

Title:Nurse communication with patients suffering from dementia
Abstract:Introduction: Dementia is a severe mental disorder and is among the most common disorders in old age, this brings many problems and hardships not only for the diseased but also for their relatives. Monitoring and caring for people with dementia can be very stressful and extremely difficult. Especially the problem in communication requires a lot of expertise, patience and persistence from nurses. Aim of this study was to determine the difficulties health professionals are facing in a conversation with demented patients, which circumstances have an influence on the conversation and identify the most common barriers that impede effective communication. Methods: The descriptive method was used for the survey and a questionnaire of the combined type as a research tool. Quantitative data were statistically analyzed using Microsoft Excel and the results illustrated in the form of pie charts and graphics loaded chart. Results: The study found that almost all employees are experiencing difficulties in communicating with a demented patient. These problems include patients not finding the right words, employees not wanting to listen and patients repeating their words. This shows that the problems occur continuously and the work has to be done consistently and professionaly. For communicationan employee calls the patient by name, using simple sentences and slow speech. For non-verbal communication eye contact, facial expressions and body language are most common among employees. We also found that employees in spite of their expertise, experience and motivation to work with dementia patients are feeling preoccupied at the end of the day, and that work organization offers too little professional training in the field of psychiatric gerontology. Discussion and Conclusions: Successful communication between nurse and demented patients is crucial for their further development of the disease. In order to successfully work with patients with dementia and their families the nurse not only has to know all of the symptoms but also the stages of dementia with associated symptoms while constantly educating herself about the disease. This is the basis for an individualized approach to care for a patient with dementia.
