Title:Uspešnost skupnostne psihiatrične obravnave oddelka za psihiatrijo

Abstract:Na oddelku za psihiatrijo smo začeli izvajati ambulantno skupnostno psihiatrično obravnavo (v nadaljevanje ASPO.) Psihiatrična rehabilitacija v skupnosti zajema: načrtovanje ciljev za izboljšanje pacientovega funkcioniranja, učenje veščin na domu, vključevanje pacientov v programe nevladnih in drugih organizacij in postopno zmanjševanje stopnje podpore. Namen raziskave je bil, ugotoviti ali je dejavnost, ki jo opravljamo v domačem okolju v okviru ASPO, dosegla svoj namen, to je zmanjšanje števila institucionalnih obravnav. Evropska unija namenja duševnemu zdravju vse več pozornosti, sprejela je vrsto strateških dokumentov in tako je slovenski zakonodajalec leta 2008 sprejel Zakon o duševnem zdravju v okviru javne službe. S podpisoma deklaracije o duševnem zdravju in akcijskega načrta za duševno zdravje v Evropi na ministrski konferenci v Helsinkih, januarja 2005, se je tudi Slovenija zavezala k implementaciji obravnave psihiatričnih pacientov v skupnosti. Gre za premik iz tradicionalne bolnišnične obravnave k obravnavi v domačem okolju, ki je posledica razvoja sodobnih pristopov k zdravljenju duševne motnje, boljše uspešnosti zdravljenja le-te ter splošne družbene osveščenosti glede duševne bolezni. Metodologija raziskovanja V raziskavi smo uporabili kvantitativno metodologijo in komparativno raziskovalno metodo. Podatki o delovanju programa ASPO ter o številu vključenih pacientov so dobljeni iz podatkovne baze bolnišničnega sistema MEDIS. V raziskavi je sodelovalo 18 pacientov, ki so eno leto vključeni v ASPO, ter so se že pred tem zdravili na oddelku za psihiatrijo. Rezultati V raziskavi smo ugotovili, da se je z vključitvijo v ASPO število hospitalizacij zmanjšalo za 57,14 %. Obiski v psihiatrični urgentni ambulanti so se zmanjšali za 68,18 %. Ležalna doba se je skrajšala za 78,15 %. Sklep Z raziskavo smo primerjali število hospitalizacij, dolžino hospitalizacij in število obiskov v urgentni psihiatrični ambulanti pred in po vključitvi v program ASPO. Število in dolžina hospitalizacij se je skrajšala, prav tako se je zmanjšal obisk v urgentni psihiatrični ambulanti. Po vseh kazalcih smo ugotovili, da je obravnava pacientov, ki so vključeni program ASPO, uspešna in kakovostna.

Title:The efficiency of the community psychiatric treatment at the psychiatric department
Abstract:In the Department of Psychiatry we started to implement the community psychiatric outpatient treatment (hereinafter: CPOT). Psychiatric rehabilitation in the community includes: planning goals for improving the patient’s functioning, learning skills at home, including patients into non-governmental and other organisations, and a progressive decrease of the level of support. The purpose of the research was to determine whether the activity we are implementing in the home environment within the framework of CPOT has reached its intent, which is a decrease in the number of institutional treatments. The European Union dedicates increasing attention to mental health. It has adopted a number of strategic documents, and in 2008 the Mental Health Act was adopted by the Slovenian legislation within the framework of public service. By signing the Mental Health Declaration and the Mental Health Action plan for Europe at the ministerial conference in Helsinki in January 2005, Slovenia also undertook to implement the treatment of psychiatric patients in the community; this represents a progress from the traditional psychiatric treatment towards the treatment in the home environment, which is a consequence of development of modern approaches to the treatment of mental disorders, a better success rate of the treatment itself and a general society awareness concerning mental illness. Research Methodology. Quantitative methodology and a comparative research method were used in the research. The data on the operation of the CPOT programme and the number of patients included in the CPOT were obtained from the MEDIS medical system. The research included 18 patients who were registered in the CPOT for one year and had been treated before in the Department of Psychiatry. Results. The research showed that the number of hospitalisations was decreased by 57.14% with the inclusion into the CPOT. The number of visits to the emergency psychiatric unit was decreased by 68.18%. The length of hospital stay was decreased by 78.15%. Conclusion. With this research we compared the number of hospitalisations, the length of hospitalisations and the number of visits to the emergency psychiatric unit before and after the inclusion in the CPOT programme. The number and length of hospitalisations has decreased, along with the number of visits to the emergency psychiatric unit. According to all indicators we established that the treatment of patients who are included in the CPOT programme is successful and of quality.
