Title:Vloga anestezijske medicinske sestre pri lajšanju pooperativne bolečine po carskem rezu s pomočjo elastomerske črpalke

Abstract:Teoretična izhodišča: Bolečina je pojav, s katerim se medicinska sestra pogosto srečuje na vseh področjih svojega delovanja. V diplomski nalogi smo opisali različne načine lajšanja pooperativne bolečine s poudarkom na lajšanju pooperativne bolečine z neprekinjeno infuzijo analgetičnih učinkovin v operativno rano, apliciranih s pomočjo elastomerske črpalke. Namen raziskave: Namen diplomskega dela je predstaviti delokrog in pristojnosti anestezijske medicinske sestre pri lajšanju pooperativne bolečine, pripravo učinkovin za neprekinjeno infuzijo analgetičnih učinkovin v operativno rano ter ugotoviti, katera analgetična mešanica najbolj učinkovito lajša pooperativno bolečino pri porodnicah po carskem rezu. Metode: V raziskovalnem delu smo izvedli klinično raziskavo. Podatke smo zbrali s pomočjo anketnega vprašalnika in ocenjevanja bolečine pri porodnicah z vizualno analogno lestvico. Anketni vprašalnik je vseboval 13 vprašanj zaprtega tipa. Rezultati in razprava: Glede na rezultate ankete so bile porodnice ustrezno seznanjene z načini lajšanja bolečine po carskem rezu, razumele so tudi pomen merjenja jakosti bolečine. Z razlago načina lajšanja pooperativne bolečine po carskem rezu z elastomersko črpalko je bilo dobro seznanjenih 18 porodnic (90 %), 2 porodnici (10 %) pa nista bili seznanjeni. Zadovoljivo lajšano bolečino je ocenilo 17 (85 %) porodnic. Porodnice, ki so dobile po elastomerski črpalki aplicirano analgetično mešanico 0,25 % levobupivakain in ketorolaka, so bile 48 ur po carskem rezu brez bolečin (1-2 po vizualni analogni lestvici).

Title:The role of the nurse anesthetist in postoperative pain management with elastomeric pump after caesarean section
Abstract:Theoretical basis: Pain is a clinical sign with which a nurse deals daily in all fields of her work. We described different ways of postoperative pain management with the emphasis on the uninterrupted infusion of analgesic substances into the operative wound with an elastomeric pump. Research goal: The aim of this study is to present the workplace and competences of a nurse anesthetist regarding postoperative pain management, drug preparation for an uninterrupted infusion of analgesic substances into the operative wound and establishing which analgesic drug is better in postoperative pain management after a caesarian section. Methods: We conducted a clinical study. Data was gathered with the help of an anonymous survey and a visual analogue scale grading pain . The survey contained thirteen closed-ended questions. Results and discussion: On the base of our results we concluded that the parturients were correctly informed about pain management after a caesarian section and understood the meaning of measuring pain intensity. 18 (90%) parturients with postoperative pain management after a caesarian section with an elastomeric pump were well-informed and 2 (10%) were considered the information unsatisfactory, despite the fact that all the patients received the same information about pain management. Pain management was satisfactory in 17 (85%) parturients. Parturients who received an elastomeric pump with the applied analgesic mixture of 0.25% Levobupivacaine and Ketorolac were without pain (1-2 on visual analogue scale) 48 hours after a caesarian section.
