Title:Preprečevanje prenosa esbl pozitivnih bakterij v domu starejših in ozaveščenost negovalnega osebja

Abstract:Diplomsko delo zajema opis bakterij, ki se pogosto pojavljajo v domu starejših, to so ESBL pozitivne bakterije odporne proti številnim antibiotikom, ki jih uporabljamo za zdravljenje okužb. Predstavili smo najpogostejše načine prenosa bolnišničnih okužb kot so: okužene roke negovalnega osebja in okuženi pripomočki za zdravstveno nego, ki predstavljajo največji problem. Opisali smo načine preprečevanja prenosa ESBL pozitivnih bakterij kot so umivanje in razkuževanje rok negovalnega osebja, uporaba medicinskih rokavic, čiščenje in razkuževanje površin ter pripomočkov za nego. Predstavili smo starostnika in njegovo bivanje v domu starejših. Za starostnika je sprememba bivalnega okolja z odhodom v domsko varstvo velik stres, zato mora negovalni tim starostnika sprejeti načrtovano s posebno individualno pozornostjo. Pomembno je tudi, da svojci tesno sodelujejo z negovalnim osebjem, kar ugodno vpliva na njihovega starostnika. Z raziskavo, ki smo jo izvajali v domu starejših občanov, smo ugotavljali poznavanje aktivnosti za preprečevanje prenosa ESBL pozitivnih bakterij. Raziskava je temeljila na kvantitativni metodologiji. Opravili smo retrospektivno analizo okužb s pomočjo zdravstvenih kartotek stanovalcev. Ugotovili smo, da se je število okužb z ESBL pozitivnimi bakterijami od leta 2008 do leta 2011 zmanjšalo. Izvedli smo tudi anketiranje s pomočjo vprašalnika, kjer smo ugotavljali koliko negovalni tim pozna pojem ESBL pozitivne bakterije, kdaj izvajamo umivanje in kdaj razkuževanje rok. Ugotovili smo, da je negovalni tim, v domu starejših občanov seznanjen s pojmom ESBL pozitivne bakterije, in da pozna ukrepe za preprečevanje le te. Ugotovili smo tudi, da si negovalni tim želi več izobraževanja na tematiko ESBL pozitivnih bakterij. Okužbam v celoti se v domu starejših ni moč izogniti, možno jih je zmanjšati na najnižjo sprejemljivo raven in sicer z izobraževanjem negovalnega osebja in doslednim izvajanjem ukrepov umivanja in razkuževanja rok ter pripomočkov.

Title:Preventiving transmission of esbl positive bacteria in an old people´s home and awareness of nursing staff
Abstract:This thesis covers the description of bacteria that often occur in nursing homes, these are ESBL positive bacteria, resistant to many different antibiotics that are used for treatment of infections. We presented the most common ways how to treat hospital infections such as: infected hands of the nursing staff and infected appliances for nursing care that present the biggest problem. We described the ways how to prevent transmission of ESBL positive bacteria, such as cleaning and disinfecting hands of the nursing staff, use of medical gloves, cleaning and disinfecting surfaces and appliances for care. We presented an elderly and his life in nursing home. It is a stress for an elderly to go to a nursing home and change the living environment so his nursing team has to accept him with a special individual attention. It is also important that relatives cooperate with the nursing staffs which have a positive effect on their elderly. With the research we performed in a nursing home we tried to determine the knowledge of activities for preventing the transformation ESBL positive bacteria. The research was founded on the quantitative methodology. We made a retrospective analysis of infections with the help of medical files of residents. We learned that the number of infections with ESBL positive bacteria was lowered from the years 2008 until 2011. We performed a questionnaire where we tried to find out how the nursing team knows the meaning of ESBL bacteria, when do we wash and disinfect hands. We learned the nursing team is aware of the meaning of ESBL positive bacteria and they know how to prevent it. We learned that the nursing staffs want to learn more on ESBL positive bacteria. We cannot avoid infections in nursing homes but it is possible to reduce them at the lowest acceptable level by educating the staff and with consistent performance of washing and disinfecting hands and appliances.
