Title:Obravnava fantomske bolečine pri pacientu s kostnim rakom po amputaciji spodnje okončine

Abstract:Spodnje okončine nam omogočajo hojo, vstajanje, pravilno držo, tek, pa tudi druge gibalne funkcije. Zraven gibalnih imajo tudi funkcijo ravnotežja, simetrije in vizualne telesne podobe posameznika. Vse omenjene funkcije pa ne morejo delovati, če je ena od spodnjih okončin amputirana. Amputacijo v približno 80% primerih spremlja fantomska bolečina ali fantomske sentacije. Za razumevanje pacientovega doživljanja je potrebno poznati delovanje centralnega živčnega sistema pri amputaciji, prav tako pa psihološke prilagoditve pacientana amputacijo. Namen diplomskega dela je opisati obravnavo fantomske bolečine pri pacientu s kostnim rakom na podlagi študije primera. Teoretični del je temeljil na deskriptivni metodi dela, izvedbo študije primera pa smo opravili s kvalitativno metodo dela. Izvedli smo intervju s pacientko po prebolelem kostnem raku z fantomskimi senzacijami. Ugotovili smo, da v kliničnem okolju posebej ne obravnavajo, ocenjujejo ali dokumentirajo fantomske bolečine, odpravljajo jo s farmakološkimi sredstvi, ali si pacienti pri lajšanju medsebojno delijo informacije ali si poskušajo pomagati sami. Amputacija in fantomska bolečina zaznamujeta človeka za celo življenje, fantomska bolečina lahko izzveni, lahko pa tudi ostane ali se pojavlja v različnih situacijah. Potrebna je prilagoditev na nov, drugačen način življenja. Pacient mora biti motiviran za zdravljenje, rehabilitacijo in čimprejšnjo samooskrbo.

Title:Treatment of phantom limb pain after amputation in patients wiht bone cancer
Abstract:Low limbs are main in walking, standing, right posture, running and in other phisical examination. They also have main function in body balance, simetry and visual body image. If one of low limbs is missing, it breaks whole operation sistem. Phantom limb pain and phantom sensations appear in 80% of amputies. Understanding patient's experience is necessary, nurses must have knowlage about operations in central nerve system in amputation. Nurses also have to know how patient psyhological feels in limb loss. The diploma paper purpose is to descript dealing with phantom limb pain in patient with bone cancer in one-case study. In theoretic part we used description work method, and qualitative work method in one-case study. Our one-case study based on interview with patient after bone cancer with phantom sensations. In clinical space there is no obvious practical deals in phantom limb pain, nurses don't managing or documenting phantom limb pain. They balanced it with pharmacological methods, in many cases pacients help each other and share information in dealing with phantom limb pain. amputation and phantom limb pain live deep seal in patients, phantom limb pain could disapear, but it also can stay or apearing for all life in main situations. Patients have to adjust in new and different kind of life. It has to be motivated in treatment, rehab and the fastest self-sufficiency.
