Abstract:	Socialna ogroženost povečuje ranljivost družbe, kadar pride do posledic večje negotovosti v dohodkovni nestabilnosti in pri zaposlitvi do povečevanja krhkosti družinske podpore, osamljenosti in občutka manjvrednosti. V diplomskem delu smo opredelili pomen socialne ogroženosti, definirali smo revščino in socialno izključenost, ki sta del socialne ogroženosti. Predstavili smo vlogo patronažne medicinske sestre pri socialno ogroženem pacientu, opisali socialno dejavnost v patronažnem varstvu, pristope patronažnega varstva in koordiniranje dela z drugimi službami in organizacijami. Metodologija raziskovanja: uporabili smo kvalitativno raziskavo. Na podlagi študije primera socialno ogrožene pacientke smo s pomočjo odprtega intervjuja, pri katerem je bilo zastavljenih osem vprašanj, pridobili osnovne podatke o pacientki in podatke o njenem zdravstvenem ter socialnem stanju. Zbrane ugotovitve smo predstavili s pomočjo analize besedila. Rezultati: iz raziskave študije primera in analize literature smo ugotovili, da je socialno ogrožena pacientka prikrajšana ne samo finančno, temveč je tudi socialno izolirana. Ugotovili smo tudi, da je patronažna medicinska sestra pacientki podala informacije o različnih službah in organizacijah (Rdeči križ, Karitas, center za socialno delo), s katerimi si lahko pomaga pri reševanju iz socialne stiske, ji svetovala različne vrste pomoči, ki bi lahko pacientki olajšale nastalo situacijo, povezala se je tudi s socialno delavko. Sklep: vloga patronažne medicinske sestre pri obravnavi socialno ogroženega pacienta je zelo pomembna, saj mora delovati humano, empatično, individualno in strokovno ter sistematično spremljati in reševati težave skupaj s pacientom. V takšni situaciji se mora povezati z drugimi službami in organizacijami, ki lahko pripomorejo k optimalni rešitvi stanja pri socialno ogroženem pacientu. Te so: center za socialno delo, Zavod za oskrbo na domu, Rdeči križ, Karitas, lokalna skupnost, dom starejših občanov.

Title:	The role of community nurses in socially endangered patients
Abstract:	Social risk increases the vulnerability of society when it comes to the effects of increased uncertainty in income instability, employment, increasing the fragility of family support, loneliness and feelings of inferiority. In the thesis we have identified the importance of social risks, what is poverty and social exclusion, which are part of the social risks. We presented the role of community nurse working with social endangered patient, described social activity in the home health and hospice care, community health nursing approaches and coordination of work with other departments and organizations. Research Methodology: we used qualitative research. Based on case studies of socially disadvantaged patient we were using an open interview, which has been set with 8 questions, obtaining basic information about the patient, and information on its health and social status. The collected findings were presented through the analysis of the text. Results: from the research case studies and analysis of the literature, we found that socially disadvantaged patient is deprived not only financially, but also socially isolated. From the case studies, which were carried out, we found that the community nurse gave the patient information on the various services and organizations (Red Cross, Caritas, Center for Social Work), which can help to solve social distress. She advised different types of assistance that could facilitate patient situation, together with the social worker. Conclusion: the role of community nurse in addressing social surveillance of the patient is very important because it must act humane, empathetic, individually and professionally and systematically monitor and solve problems together with the patient. In such a situation she must integrate with other services and organizations that can contribute to an optimal solution to the situation in the social endangered patient. These are: the Centre for Social Work, Institute for home care, the Red Cross, Caritas, the local community, homes for the elderly.
