Title:Osamitev in karakterizacija bakterije Clostridium difficile iz vzorcev poplavne vode in prsti

Abstract:Clostridium difficile je patogena bakterija, ki povzroča črevesne bolezni pri ljudeh in živalih. Okužbe s C. difficile sodijo trenutno med najpogostejše bolnišnične črevesne okužbe v številnih državah. Število obolelih v zadnjih letih namreč vztrajno narašča. Raziskave o C. difficile v izvenbolnišničnem okolju so maloštevilne, zato smo v magistrskem delu proučevali prisotnost in tipe bakterije C. difficile v okoljskih vzorcih: v vzorcih poplavne vode in prsti iz različnih okolij (njiva, travnik, gozd, kompost, pašnik in skladišče drv).C. difficile smo iz vzorcev vode in prsti osamili na selektivnih gojiščih. Vzorce prsti smo testirali z dvema metodama, ki sta se razlikovali v načinu priprave suspenzije prsti (brez namakanja in z namakanjem). Izolate C. difficile smo karakterizirali z genotipizacijskima metodama toksinotipizacijo in PCR-ribotipizacijo. Analizirali smo 44 vzorcev poplavne vode in 44 vzorcev prsti. Pozitivnih je bilo 10 vzorcev vode (22,7 %), iz katerih smo pridobili 73 izolatov C. difficile. Iz skupno 16 pozitivnih vzorcev prsti (36,4 %) smo izolirali 178 izolatov. Izolate smo uvrstili v 21 različnih PCR-ribotipov in 5 toksinotipov, med katerimi sta toksinotipa IXb in XId nova. Skoraj polovica PCR-ribotipov iz vode in prsti se je ujemala s PCR-ribotipi, ki jih najdemo pri ljudeh in živalih v Sloveniji. Naši rezultati potrjujejo, da je C. difficile pogost tudi v izvenbolnišničnem okolju. Pokazali smo, da se genotipi C. difficile v okolju delno prekrivajo z genotipi, ki se pojavljajo pri živalih in ljudeh, kar kaže, da prst in luže lahko predstavljajo enega od rezervoarjev bakterije C. difficile. Kljub omenjenemu prekrivanju genotipov najdemo v zemlji in lužah tudi veliko genotipov, ki pri človeku in živalih še niso bili opisani.

Title:Isolation and characterisation of Clostridium difficile from flood water and soil samples
Abstract:Clostridium difficile is human and animal pathogen associated with intestinal infections. C. difficile is currently among the most frequent nosocomial intestinal pathogens in many countries and number of cases is increasing. As the number of studies on C. difficile in the environment is limited, we focused on the occurrence of C. difficile in environmental samples. We examined the prevalence and genotypes of C. difficile in samples of puddles and soil from different environments (fields, meadows, forests, compost, pastures and firewood storage). C. difficile was isolated on selective media and characterized with toxinotyping and PCR-ribotyping. In total 44 puddle samples and 44 soil samples were analysed. From 10 positive puddle samples (22,7 %) 73 C. difficile isolates were obtained. From 16 positive soil samples (36,4 %) 178 C. difficile isolates were obtained. Isolates were distributed into 21 different PCR-ribotypes and 5 toxinotypes, among which toxinotype IXb and XId are new. Almost a half of PCR-ribotypes from puddles and soil were already detected in humans and animals in Slovenia. Our results demonstrated that C. difficile is widespread also in nonhospital environment. An overlap of C. difficile genotypes found in environment, humans and animals indicates the possibility that nonhospital environment could be one of the reservoirs of C. difficile. Despite this overlap many genotypes found in soil and puddles have not yet been isolated from humans and animals in Slovenia. Differences of genotypes were also observed between puddles and soil.
