Abstract:V diplomskem delu smo obravnavali uporabo metode drevo odpovedi v kemijskih procesih.

Title:FAULT TREE ANALYSIS IN CHEMICAL PROCESSES
Abstract:This diploma thesis discusses the use of the Fault Tree Analysis (FTA) in chemical processes. This method provides a sequence of causes and unpleasant events that may lead to accidents. Using this method, the risks of uncontrolled events were identified, such as the equipment failures, poor maintaince of equipment, insufficient qualifications of workers, errors in the production processes, and the hazards to humans. We sought for free-available software by which the method could be applied to several case studies. The availability of databases and the methods to provide frequencies and probabilities of failures for various process equipment and systems were also reviewed. This thesis demonstrates the whole procedure for building a fault tree, obtaining input data, and quantifying the tree following by an interpretation of the results. The computer program PTC Windchill FTA was used in this thesis for quantifications of fault trees. The applications of the procedure are illustrated by three case studies: the release of ammonia from an absorption refrigeration system, an explosion of pressure vessel, and a boiling liquid expanding vapour explosion (BLEVE). In this way, a qualitative insight into the causes and the propagation scenarios that lead to these events were acquired as well as the quantitative assessments of the frequencies and probabilities.
Keywords:fault tree analysis (FTA), human errors, minimal cut set, frequency, probability
