Abstract:Teoretična izhodišča. Sklep.

Abstract:Theoretical background. Important factors in health care between the community nurses and the chronically ill are communication and a good relationship. Because of the increasing number of patients with advanced chronic diseases who want to stay in home care, community nurse has an important role in providing quality health care for patients. Research Methodology. In this thesis, we used a descriptive method of work and quantitative research methodology. For research instrument we used a questionnaire. The research sample consisted of 30 elderly people aged between 65 and 92 years. Data were collected by community nurses, who have interviewed their patients with 17 structured questions during visits of curative care. Results. The results showed that the elderly are happy in relation to community nurse. In the health care of elderly people at home are important communication, information on the treatment and quality attitude of community nurses. Although the disease affects the quality of life of elderly, in spite of these diseases, they do not want to talk about the process of aging and death. Conclusion. Attitude of the elderly toward community nurses is positive and of good quality. Home living and help-needy elderly people are eager of curative care visits by community nurses because they are talking to them and ease the development of the disease. Her role is important in raising awareness of chronically ill older people about palliative care, because it is within the entire health system.
