Title:Posebni primeri merjenja neinvazivnega krvnega tlaka po avskultacijski metodi

Abstract:Izhodišča in namen: Neinvazivno merjenje krvnega tlaka spada v pogosto izvajan monitoring obtočil v kliničnem okolju. Med zaposlenimi medicinskimi sestrami smo raziskali poznavanje posebnih primerov pri merjenju avskultacijske vrzeli in odsotnost pete faze Korotkovih tonov. Raziskali smo tudi teoretično ozadje teh fenomenov. Namen naše naloge je bil ugotoviti, ali medicinske sestre ta dva fenomena poznajo ter kolikokrat so se z omenjenima fenomenoma že srečale v kliničnem okolju. Raziskovalna metodologija in metode: Raziskava je temeljila na kvantitativni metodologiji. V priložnostnem vzorcu smo zaposlenim medicinskim sestram ponudili več scenarijev v katerih so morale izmeriti krvni tlak po avskultacijski metodi na simulatorju. Simulirali smo normalni krvni tlak, avskultacijsko vrzel, hipertenzijo in odsotnost pete faze po Korotkovu. Rezultati: Ugotavljamo, da medicinske sestre, ki delajo v kliničnem okolju, avskultacijsko vrzel prepoznajo v simuliranem okolju v 75,8 %. Pojavnost odsotnosti pete faze po Korotkovu pa prepoznajo v 36,4 % v simuliranem okolju. V kliničnem okolju se je pet medicinskih sester že srečalo s pojavom avskultacijske vrzeli in devet medicinskih sester z odsotnostjo pete faze po Korotkovu pri merjenju krvnega tlaka po avskultacijski metodi. Diskusija in zaključek: Primera se sicer redko pojavita pri merjenju tlaka, vendar to ne more opravičiti njihovega nepoznavanja. Medicinske sestre v kliničnem okolju ta dva fenomena še redkeje srečujejo, ker se krvni tlak vse pogosteje meri s samodejnimi merilniki krvnega tlaka. Če zgrešimo avskultacijsko vrzel, napačno določimo sistolično vrednost krvnega tlaka, ki je podcenjena.

Title:Specific cases of non-invasive blood pressure measurement using the auscultatory method
Abstract:Purpose: Non-invasive blood pressure measurement is one of the often-utilised procedures of circulatory measurement in a clinical environment. It has been examined how much healthcare personnel know about specific cases of measurement: auscultatory gaps and the absence of the fifth Korotkoff sound. The theoretical background of these phenomena has also been investigated. The purpose of the present thesis has been to discover whether healthcare personnel is familiar with the two phenomena and how many times they have encountered them in a clinical environment. Methodology: Quantitative methods were used for this study. In the purposive sample the healthcare personnel were given several situations where they had to measure blood pressure using the auscultatory method through a simulator. Normal blood pressure, auscultatory gap, hypertension, and the absence of the fifth Korotkoff sound were simulated. Results: It has been discovered that healthcare personnel working in a clinical environment recognize the auscultatory gap in 75,8% of cases in a simulated environment. The absence of the fifth Korotkoff sound is recognized in 36,4% of cases in a simulated environment. In a clinical environment five of the healthcare staff have encountered the auscultatory gap and nine have encountered the fifth Korotkoff sound when using the auscultatory method for blood pressure measurement. Discussion and conclusions: The cases are rarely found in blood pressure measurement but that should not excuse poor knowledge of them. The two phenomena are even less frequently encountered by the healthcare staff working in a clinical environment since the blood pressure is increasingly frequently measured by automatic blood pressure monitors. By missing an auscultatory gap the systolic blood pressure can be set too low.
