Abstract:Zlom nosne kosti je tretji najpogostejši zlom med vsemi zlomi. Kot vsak zlom tudi tega spremlja huda pred in pooperativna bolečina. Naloga zdravstvenega osebja, tako kirurgov kot medicinskih sester je, da pravočasno prepoznajo in nato obvladujejo bolečino in s tem pripomorejo k večjemu zadovoljstvu pacienta nad samo oskrbo. Bolečina je razširjen problem pri zlomu nosu. Pomembno je, da se pravočasno ugotovi in obvladuje bolečino. Gre za timsko delo zdravnikov in medicinskih sester. Danes se uporablja različna medikamentozna sredstva že v času izvajanja operativnega posega, ki še imajo potem dober učinek v pooperativnem obdobju in s tem zmanjšajo porabo opioidnih sredstev in ostalih analgetikov. Pri analizi strokovne literature smo ugotovili, da pri pacientu po operaciji nosu povzroča hudo bolečino nosna tamponada, a ne le ob vstavitvi, temveč tudi po odstranitvi tamponov. Bolečina je tudi manjša pri uporabi kombinacije pregabalina in deksametazona. S tem je zmanjšana uporaba ostale opioidne terapije. Tudi infuzija esmolola zmanjša bolečino v prvih treh urah po operaciji. Najpogosteje uporabljeni bolečinski lestvici za ocenitev akutne pooperativne bolečine po obraznem posegu sta VAS in NRS.

Title:Nasal bone fracture
Abstract:Nasal bone fracture is the third most common fracture. Like anyother fracture this one is also accompanied by a severe pre and postoperative pain. It's on nurses and surgeons to evaluate and to control pain and to make sure that patient is comfortable and happy with the care he/she receives. Pain is a big problem in nasal bone fracture. It's important to evaluate and to control it on time. Here it comes to a team work between doctors and nurses. In today's practice are already in use different medicament treatment. The medications that are used are also efficiant in postoperative period. And because of that it comes to decreased use of opioids and other analgetics. We found out that the most pain in patients after the surgery of nasal bone fracture is caused by nasal packings, not only when they are inserted but also after the removal. Pain is decreased when pregabalin and dexamethason are used as a combination. With that is also decreased use of opioids. Also esmolol infusion decreases pain in first three hours after the surgery. And that the best pain measurement tools for evaluation of acute maxillofacial postoperative pain are VAS and NRS.
