Title:Management razjed zaradi pritiska v akutni kirurški obravnavi pacienta

Abstract:Izhodišča: Management RZP je del zdravstvene nege, ki zahteva poznavanje dejavnikov tveganja, principov ocene tveganja, preprečevanja, identifikacije in zdravljenja. Celostni pristop k preprečevanju in obravnavi RZP od izvajalcev zdravstvene nege zahteva dodatna znanja in izkušnje. Namen magistrskega dela je opredeliti in opisati management RZP v akutni kirurški obravnavi pacienta. Metode: Raziskava je temeljila na kvantitativni metodologiji raziskovanja. Izvedli smo deskriptivno študijo z metodo anketiranja. Kot raziskovalni instrument smo uporabili anketni vprašalnik, ki smo ga sestavili na podlagi izsledkov literature. Za prikaz rezultatov smo uporabili metodo deskriptivne statistike in metode inferenčne statistike. Za potrditev hipotez smo uporabili Spearmanov koeficient korelacije, T –test in Mann-Whitney test. Rezultati: Rezultati so pokazali, da je med spremenljivkama ovire za nedosledno izvajanje preventivnih ukrepov ter dejavniki organizacije korelacija šibka, negativna (rs=-0,129; p=0,212). Mann-Whitney test je pokazal, da obstajajo statistično pomembne razlike med delovno dobo in preventivnim delovanjem. Izvajalci zdravstvene nege z daljšo delovno dobo v povprečju višje ocenjujejo področje preventivnega delovanja pri RZP (Z=-2,919; p=0,004). Ugotavljamo tudi, da med izvajalci zdravstvene nege z dodiplomsko in podiplomsko izobrazbo ter tistimi s srednješolsko izobrazbo obstajajo statistično pomembne razlike pri prepoznavi preventivnih ukrepov in obravnavi pacienta z RZP (Z=-2,418; p=0,016). Diskusija in zaključek: RZP je pomemben kazalnik kakovosti zdravstvene nege, vendar kljub napredku, obstoju smernic in novih znanj ostaja pojavnost RZP nespremenjena že več desetletij. Management RZP v akutni kirurški obravnavi pacienta zajema ocenjevanje tveganja za nastanek RZP, preventivno delovanje, v primeru pojava RZP pa temeljito multidisciplinarno obravnavo pacienta z RZP. Izobraževanje izvajalcev zdravstvene nege na področju preventivnega delovanja je ključnega pomena in predstavlja temeljno aktivnost zdravstvene nege v obravnavi RZP.

Title:Management of pressure ulcers in the acute surgical treatment of the patient
Abstract:Objectives: Ulcer pressure management is a part of nursing that requires knowledge of risk factors, principles of risk assessment, prevention, identification and treatment. An integrated approach to preventing and treating pressure ulcer from nursing care providers requires additional knowledge and experience. The purpose of this master's thesis is to define and describe management of ulcers due to pressure in the acute surgical treatment of the patient. Methods: The research was based on quantitative research methodology. A descriptive study with the survey questionnaire, compiled on the basis of literature results and used as a research instrument, was conducted. The Spearman coefficient of correlation, the T-test and the Mann-Whitney test were used to confirm the hypothesis. Results: The results showed that among the variables, the obstacles to the inconsistent implementation of preventive measures and organizational factors had a weak, negative correlation (rs =-0,129; p=0,212). The Mann - Whitney test showed statistically significant differences between service life and preventive action when dealing with pressure ulcer. Nursing workers with longer years of service assessed on average higher the area of preventive activity in pressure ulcer (Z=-2.919; p=0.004). There were statistically significant differences between health care providers with undergraduate and postgraduate education and those with secondary education in identifying preventive measures and treating the patient with pressure ulcer (Z=-2.418; p=0.016). Discussion and conclusion: Pressure ulcer is an important indicator of the quality of nursing care, but despite the progress, established guidelines and greater knowledge, the pressure ulcer's incidence remains the same for several decades. Management of pressure ulcer in acute surgical treatment of a patient includes risk assessment for the occurrence of pressure ulcer, preventive action and in the case of pressure ulcer the thorough multidisciplinary treatment. Education of nursing care providers in the field of preventive actions is crucial and presents the basic activity of nursing care in the treatment of pressure ulcer.
