Title:Odpust pacienta iz bolnišnice in nadaljevanje zdravstvene obravnave v patronažnem varstvu

Abstract:Izhodišče in namen: Ležalna doba v bolnišnici se krajša, pacienti so vedno hitreje odpuščeni v domače okolje, kjer mnogi potrebujejo nadaljevanje zdravstvene obravnave. Tudi odpust novorojenčka in otročnice pogosto ni javljen patronažni službi, saj se mnogo porodnišnic sklicuje na varstvo osebnih podatkov. Namen raziskave je ugotoviti kako poteka kontinuirana zdravstvena obravnava v patronažnem varstvu po odpustu pacienta iz bolnišnice ter otročnice in novorojenčka iz porodnišnice. Raziskovalne metode: Uporabljena je bila kvalitativna metodologija raziskovanja. V raziskavo smo vključili 7 diplomiranih medicinskih sester v enem izmed zdravstvenih domov na Štajerskem v enoti patronažnega varstva. Podatke smo obdelali po metodi analize vsebine, ki smo jih predhodno dobili s pomočjo pol strukturiranih intervjujev ter jih opisno predstavili. Rezultati: Ugotovili smo, da kontinuiteta zdravstvene obravnave po odpustu pacienta oz. otročnice in novorojenčka v domače okolje ni vedno zagotovljena. Pogosto prihaja do prekinitve informacij oz. so le-te pomanjkljive, kar vpliva na delo diplomirane medicinske sestre v patronažnem varstvu ter pacienta oz. otročnico in novorojenčka in njihovo zdravstveno stanje. Diskusija in zaključek: Potrebno bi bilo urediti enotno dokumentacijo ob odpustu, nanašajočo se na zdravstveno nego ter oskrbo ali še boljše, urediti informacijski sistem po vsej državi. Enotni informacijski sistem bi omogočal vpogled vsem zdravstvenim delavcem na vseh ravneh, zaposlenim v različnih ustanovah ter preprečeval izgubo informacij.

Title:Patient's discharge from hospital and continuation of health treatment in home care
Abstract:Basis and aim: The length of the inpatient stay is getting shorter, but many patients still require continued health care. Furthermore, the discharge of a child and their neonatal mother is frequently not reported to the community nursing service, as many maternity hospitals refer to the protection of personal data. The aim of the research was to investigate the course of continued health care in community nursing after the discharge of a patient from the hospital, and the neonatal mother and her child from the maternity hospital. Research methods: The method used in this research was a qualitative method of investigation. It included seven community nurses working in one of the health centers in Styria that encounter patient, neonatal mother, and child discharges daily. The data was collected using half-structured interviews and then processed using the method of content analysis. Results: We discovered that the continuity of health care after the discharge of a patient or a child and their neonatal mother into a home setting is not always guaranteed. Frequently, there is a lack of information, or the information is not sufficient. This affects the community nurse’s work, the patient or the neonatal mother and her child, and their health status. Discussion and conclusion: Unified documentation concerning the patient’s health status at discharge should be implemented; or even better, an information system throughout the whole country that would allow insight to all health care workers, working at all levels in different institutions, and would, therefore, prevent loss of information.
