Title:Holistični pristop k obvladovanju pojava bakterije Legionella pneumophila v pitni vodi v izbrani bolnišnici skozi prizmo dialektične teorije sistemov

Abstract:Zdravstvene ustanove imajo zelo velike in zapletene interne vodovodne sisteme, ki so zelo dobro okolje za razmnoževanje patogenih mikroorganizmov, ki povzročijo razvoj infekcij pri imunsko oslabljenih bolnikih. Bolezni, ki se prenašajo z vodo, se širijo z zaužitjem okužene vode in tudi z inhalacijo okuženih aerosolov. Raziskave dokazujejo, da so neizprane gibljive cevi prh zelo dobro gojišče za rast in razvoj patogenih mikroorganizmov. Patogeni mikroorganizmi v gibljivih ceveh so potencialno zdravstveno tveganje za bolnika pri prhanju. Respiratorna okužba z bakterijo Legionella spp. in njeno nadaljnje razmnoževanje v alveolarnih makrofagih pljuč spodbudi razvoj legionarske bolezni in posledično razvoj pljučnice, ki je lahko smrtna. S holističnim pristopom obvladovanja izbruha bakterije Legionella pneumophila v pitni vodi in preprečevanja nadaljnjega širjenja biofilma po internih vodovodnih sistemih smo razvili algoritem procesnih ukrepov in ga implementirali v izbrano bolnišnico. Za izbiro najustreznejših preventivnih fizikalnih ukrepov smo uporabili aplikativno metodo po postopku NOVOST (angl. SREDIM): N – nabor ukrepov, O – opis ukrepa, V – vrednotenje ukrepa, O – odločitev za ukrep, S – sprememba starega stanja, T – trajnost ukrepa. Gre za orodje, t. i. vprašalnik, ki ga izpolnijo proizvajalci in/ali distributerji fizikalnih ukrepov. Primerjanje izpolnjenih vprašalnikov za isti ali podoben fizikalni ukrep odločevalcem omogoča enostavnejšo, hitrejšo in učinkovitejšo izbiro ustreznega fizikalnega ukrepa.

Title:A holistic Approach to Controlling the Bacteria Legionella pneumophila in Drinking Water in a Selected Hospital through the Prism of Dialectical Systems Theory
Abstract:Healthcare facilities have extensive and complex internal plumbing systems, which could represent a high risk of infection by pathogenic microorganisms in immunocompromised patients. Waterborne diseases are spread by the ingestion of infected water and by the inhalation of infectious aerosols. Research shows that non-washed flexible shower tubes are an excellent living environment for the growth and development of pathogenic microorganisms. Pathogens in flexible tubes are a potential health risk for the patients using the shower. Respiratory infection with Legionella spp. and its continued reproduction in alveolar lung macrophages stimulates the development of Legionnaires' disease and, consequently, the development of potentially lethal pneumonia. To eliminate and control the occurrence of Legionella spp. in the internal water supply system, an innovative holistic approach through the prism of the discourse of Dialectical Systems Theory was used, by using the SREDIM method: S – select measure, R – record measure, E – evaluate measure, D – determine (and develop) solution, I – implement the selected solution and, lastly, M – maintain the new solution. Physical measures were selected by using a tool for selecting the most suitable measure. The tool is based on a questionnaire. Every potential distributer complete this questionnaire. By obtaining completed questionnaires from different distributors for the same physical measurement, we could more easily and quickly select the most effective measure.
