Title:Aktivnosti medicinske sestre pri odpustu pacienta po poškodbi ledvenega dela hrbtenice

Abstract:V diplomskem delu smo opisali aktivnosti medicinske sestre pri pripravi na odpust pacienta po poškodbi ledvenega dela hrbtenice tekom hospitalizacije in na dan odpusta ter vključevanje svojcev skozi hospitalizacijo. Z raziskavo smo želeli ugotoviti katere negovalne diagnoze se pojavijo pri pripravi pacienta na odpust in kakšno vlogo imajo svojci pri pripravi na odpust pacienta. V teoretičnem delu smo uporabili deskriptivno metodo dela in vključili izbrano literaturo. V empiričnem delu smo uporabili kvalitativno metodologijo. Izvedli smo študiji primera. S pomočjo intervjuja in vprašalnika glede na 11 funkcijskih vzorcev po Marjory Gordon, smo izpostavili negovalne diagnoze. V študiji primera je sodelovala pacientka s poškodbo ledvenega dela hrbtenice s pridruženo poškodbo leve goleni.S pomočjo študije primera smo prišli do ugotovitve, da ima vključevanje svojcev pri načrtovanju odpusta veliko vlogo ter, da je pomembno njihovo vključevanje že pri sprejemu in tekom hospitalizacije. Pri obravnavani pacientki smo ugotovili osem negovalnih diagnoz, ki so nam pomagale, da smo izdelali načrt zdravstvene nege in s tem pacienta ter njegove svojce še bolje pripravili na sam odpust. Vsaka poškodba in sprejem pacienta v bolnišnico zanj in za svojce predstavlja stres. Zato je pomembno, da jih vključimo v zdravstveno vzgojni proces že ob sprejemu in jih tekom hospitalizacije spodbujamo, motiviramo in jih tako postopoma pripravimo na odpust. Veliko vlogo igra dobra komunikacija tako v timu, kot z pacientom in njegovimi svojci.

Title:Nurse's activities at discharge of patients after the lumbar spine injury
Abstract:In the diploma thesis we describe the activities of a nurse in preparation for the discharge of a patient after an injury of the lumbar spine through hospitalization and on the day of discharge, and the involvement of relatives throughout the hospitalization. With the survey we wanted to determine which nursing diagnoses can occur during the preparing of the patient for discharge, and what role the relatives play in preparing for the patient's discharge. In the theoretical part we used on the descriptive method of work, and have included a selected literature. In the empirical part, we have used a qualitative methodology. Through interview and questionnaire based on 11 functional samples according to Marjory Gordon, we exposed nursing diagnosis. With the help of a case study, we have come to the conclusion that the involvement of families in planning for discharge has a large role. We have eight nursing diagnoses that are helpful to us, we were able to produce a plan of nursing care and that the patient and his family members are even better prepared for discharge. Every injury and the admission to the hospital represents a stress to the patient. Therefore, it is important that they are involved in the health educational process at the admission and during the hospital stay encouraged and motivated and gradually prepare for the discharge. Good communication plays a big role in the team as well as with the patient and his or her relatives.
