Title:Poznavanje, dojemanje in odnos zdravstvenega osebja do paliativne oskrbe pri pacientih s srčnim popuščanjem na urgenci in kardiološkem oddelku

Abstract:Izhodišča: Pacienti z akutnim srčnim popuščanjem pogosto iščejo pomoč na urgenci, kjer se zdravstveni delavci s takšno boleznijo srečujejo v zgodnjem ali poznem oz. zadnjem stadiju in prav pri slednjem, se morajo bolj osredotočiti na nudenje paliativne oskrbe takšnim pacientom. Namen zaključnega dela je ugotoviti, kakšno je poznavanje, dojemanje in odnos zdravstvenega delavca na urgenci in kardiološkem oddelku do paliativne oskrbe pri pacientih s srčnim popuščanjem, ter morebitne ovire. Metode: Uporabili smo kvantitativno metodologijo z metodo anketiranja. Pri tem smo izvedli presečno deskriptivno raziskavo, ki je potekal meseca avgusta 2019. Rezultati: S pomočjo raziskave, smo ovrgli hipotezo 1, da med urgenco in kardiološkim oddelkom obstajajo statistično signifikantne razlike v znanju izvajanja paliativne oskrbe med zdravstvenim osebjem in hipotezo 2, da zdravstveno osebje na urgenci prepozna več ovir za nudenje paliativne oskrbe v primerjavi z zaposlenimi na kardiološkem oddelku, saj je bila statistična signifikanca pri vseh vprašanjih večja od 0,05. Hipotezo 3, smo delno potrdili, saj obstaja le eno stališče, kjer je bila ugotovljena povezava. To stališče je bilo: »Kardiologija, splošna medicina in interna medicina se lahko učijo iz strokovnega znanja o medicini za paliativno oskrbo« (p<0,05). Diskusija in zaključek: Menimo, da je poznavanje paliativne oskrbe na kardiološkem oddelku in urgentnem centru dobro in ne obstajajo večje razlike. Čeprav pa bodo za izboljšanje poznavanja, dojemanja in odnosa zdravstvenega osebja do paliativne oskrbe pri pacientih s srčnim popuščanjem, še potrebna dodatna izobraževanja.

Title:Knowledge, perceptions and attitudes of health care practitioners towards palliative care in patients with heart failure in the emergency room and cardiology ward
Abstract:Background: Acute heart failure is a problem, with which the public health is facing all around the World. Despite the ever improving health care system, and the resulting treatment, the incidence and the frequency of the disease is increasing year by year. Therefore, patients with acute heart failure often seek help in an emergency center, where healthcare providers encounter such a disease in the early or late stage and right at the last stage, they need to focus more on providing palliative care to such patients. Methods: We used quantitative methodology using the survey method. In doing so, we conducted a cross-sectional descriptive survey that took place in August 2019. Results: Through research, we have rejected the hypothesis 1, that there are statistically significant differences between the emergency department and the cardiology ward in the knowledge of palliative care among healthcare staff and hypothesis 2, that the emergency room health staff recognizes more barriers to palliative care compared to those in the cardiology department, as the statistical value for all questions was greater than 0.05. Hypothesis 3 was partially confirmed, as there is only one position where a correlation was found. That position was: »Cardiology, general medicine and internal medicine can all learn from palliative care medicine expertise« (p < 0.05). Discussion and conclusion: We believe that the knowledge of palliative care in the cardiology department and the emergency department is good and there are no major differences between the knowledge in the wards. However, further training will be required to improve the knowledge, perceptions and attitudes of healthcare staff towards palliative care in patients with heart failure.
