Title:Vzdržen sistem financiranja zdravstva

Abstract:Sodobni zdravstveni sistemi se soočajo s številnimi izzivi, ki med drugim ogrožajo dolgoročno vzdržnost njihovega financiranja. Slovenski zdravstveni sistem pri tem ni nobena izjema. Magistrskega dela smo se lotili z osrednjim namenom ugotavljanja ključnih slabosti financiranja slovenskega zdravstva. Poiskali smo razloge za njihov obstoj in ugotovili, na kakšen način bi se dolgoročna fiskalna vzdržnost slovenskega zdravstva lahko izboljšala. Za celovito obravnavo problema smo postavili teoretične okvirje, ki se navezujejo na zdravstveno varstvo in zdravstveni sistem. Vključili smo tudi izzive vzdržnosti, s katerimi se v sodobni družbi srečujejo zdravstveni sistemi. Poudarili smo pomen centralizacije in decentralizacije zdravstvenih sistemov. Ker se zdravstveni sistemi skozi zgodovino na podlagi reform razvijajo, smo pojasnili tudi štiri teoretično čiste oblike modelov zdravstva. Primerjali smo zdravstvene sisteme štirih evropskih držav, in sicer Slovenije, Avstrije, Nemčije ter Švedske. Opisali smo njihovo ureditev in organizacijo ter sisteme financiranja, s fokusom na zdravstvenih virih in zdravstvenih izdatkih. Navedli smo tudi največje izzive in aktualne reforme izbranih zdravstvenih sistemov. Tako smo dobili celovit pregled in izhodišča za preverjanje naših hipotez. Ugotovili smo, da se, v skladu z globalnim trendom, vse štiri opazovane države srečujejo s problematiko staranja prebivalstva, zato so primorane povečevati zdravstvene izdatke, kar je za prebivalce Slovenije še posebej obremenjujoče. Vzdržnost slovenskega zdravstva je med drugim slabša zaradi strukture virov financiranja. Prav tako smo potrdili, da je slovenska zakonodaja, ki ureja dolgotrajno oskrbo, neurejena in pomanjkljiva, hkrati pa je pri nas dolgotrajni oskrbi namenjenih bistveno manj zdravstvenih izdatkov kot v ostalih državah. Rešitve za vzpostavitev dolgoročne fiskalne vzdržnosti slovenskega zdravstva smo poiskali v izbranih državah in njihovih aktualnih reformah. Čeprav se posamezni zdravstveni sistemi in njihovi mehanizmi delovanja med seboj razlikujejo, se jih dandanes veliko srečuje s podobno problematiko. Vsekakor je primere dobre prakse smiselno poiskati tudi izven meja domačega zdravstva. Nekatere tuje politike financiranja zdravstva se lahko, sicer prilagojene specifikam slovenskega zdravstvenega sistema, zagotovo prenesejo tudi na domača tla.

Title:Sustainable system of health care financing
Abstract:Nowadays, healthcare systems face several challenges that could jeopardize their long-term fiscal sustainability. The Slovenian healthcare system is no exception. The purpose of this thesis is identifying key weaknesses in Slovenian healthcare financing and reasons behind their existence, as well as defining viable options for improvement. In order to address the problem comprehensively, we first established a theoretical framework related to healthcare and healthcare systems. Then, we described sustainability challenges that modern healthcare systems face. Furthermore, we stressed the importance of centralisation and decentralisation of healthcare systems. Since every healthcare system is unique and has been evolving throughout history, we further explained the main theoretical forms of healthcare models. The healthcare systems of four European countries were compared; Slovenia, Austria, Germany and Sweden. We described the organisation of their healthcare systems, especially in terms of financing, with an emphasis on healthcare resources and healthcare expenditures. Additionally, we described their biggest challenges and listed their most current reforms related to healthcare. Finally, with this comprehensive overview, the basis for validating our hypotheses were set. In line with the global trend, all four countries are facing challenges related to aging of the population. Consequently, their healthcare expenditure is increasing, which is especially burdensome for the citizens. It was found that the long-term sustainability of Slovenian healthcare system is compromised due to the structure of healthcare financing resources. In addition, long-term care is one of the most underdeveloped fields in the Slovenian healthcare system. Not only does it lack relevant policies, the share health expenditures spent on long-term care is visibly smaller compared to other countries included in this study. Even though every healthcare system is unique, most of them face similar challenges. Therefore, solutions for long-term fiscal sustainability can be found outside Slovenian borders. There are certainly some foreign healthcare financing policies that would, adapted to Slovenian healthcare system specifics and utilized accordingly, largely improve long-term fiscal sustainability of our healthcare.
