Title:Uporaba oralne kontracepcije kot dejavnik tveganja za nastanek raka na materničnem vratu

Abstract:Rak materničnega vratu (RMV) še vedno predstavlja pogosto vrsto malignih obolenj pri ženskah, kljub učinkovitemu programu presejanja v Sloveniji in po svetu. Dejavnik tveganja, ki lahko povzroči RMV, je poleg humanih papiloma virusov (HPV) še uporaba oralne hormonske kontracepcije (OHK). Namen zaključnega dela je razložiti mehanizem delovanja OHK kot dejavnik tveganja za nastanek predrakavih ali rakavih sprememb materničnega vratu ter opisati pomen zdravstvene vzgoje na področju preprečevanja RMV in svetovanje medicinske sestre pri izbiri kontracepcijske metode. Metode: Uporabili smo deskriptivno in komparativno metodo dela, metodo kompilacije in analizo vseh v raziskavo vključenih virov, ki smo jih iskali v različnih podatkovnih bazah, in sicer: PubMed, Medline, CINAHL ter COBISS.SI. Zastavili smo si vključitvena in izključitvena merila glede na postavljeno raziskovalno vprašanje in predstavili potek sistematičnega iskanja in pregleda virov s PRISMA modelom. Rezultati: Pregledali in analizirali smo 17 člankov, objavljenih v angleškem jeziku. Na podlagi omenjenih je bilo mogoče ugotoviti, da dolgotrajna uporaba OHK (≥5 let), ob sočasni okužbi s HPV, povečuje verjetnost za nastanek sprememb materničnega vratu, povezanih z visoko rizičnimi onkogenimi HPV. OHK je lahko tudi samostojni dejavnik tveganja, povezan z malignimi spremembami materničnega vratu, vendar verjetno posredni (več okužb s HPV). Razprava in sklep: Uporaba OHK velja za varno in učinkovito metodo kontracepcije, vendar le ob doslednem vodenju in pravilni uporabi. Medicinska sestra je dolžna izvajati zdravstveno vzgojo žensk o možnostih učinkovite preventive pred nastankom RMV in svetovati pri izbiri kontracepcije z edukacijo o prednostih in slabostih določene metode.

Title:The use of oral contraceptives as a risk factor for cervical cancer
Abstract:Cervical cancer (CC) still represents a common type of malignancy in women, despite an effective screening program in Slovenia and around the world. In addition to human papillomavirus (HPV), the use of oral hormonal contraception (OHC) is a risk factor that can cause CC. The purpose of the conclusion is to explain the influence of OHC as a risk factor for precancerous or cancerous changes of the cervix and to describe the importance of health education in the field of CC prevention and advice in choosing a contraceptive method given by the nurse. Methods: We used the descriptive and comparative method, the method of compilation, and analysis of all sources included in the research, which we found in various databases, namely: PubMed, Medline, CINAHL and COBISS.SI. We chose inclusion and exclusion criteria according to the research question and presented the course of systematic search and review of sources with the PRISMA model. Results: We reviewed and analyzed 17 articles published in English. It could be concluded that long-term use of OHC (≥5 years), together with a HPV infection, increases the likelihood of developing oncogenic types of HPV. OHC can also be a stand-alone risk factor that causes cervical malignancies. However, OHC is likely an indirect risk factor. Discussion and conclusion: The use of OHC is considered a safe and effective method of contraception, but only with consistent management and proper use. The nurse is obliged to participate in the health-educational work with women, to make them aware of the possibilities of effective prevention against the development of CC and to advise on the choice of contraception by educating about the advantages and disadvantages of a particular method.
