Title:Kazalniki preživetja pri pacientih z zunajbolnišničnim srčnim zastojem

Abstract:Uvod: Na preživetje bolnikov po zunajbolnišničnem srčnem zastoju vplivajo pristopni čas, etiologija srčnega zastoja, izvajanja TPO z uporabo AED, predhodna obolenja, onesnaženost ozračja ter izražena laktacidoza po vzpostavitvi spontane cirkulacije. Metode: V teoretičnem delu smo uporabili deskriptivno metodo dela s pregledom literature. V empiričnem delu smo zbirali in analizirali podatke o zunaj bolnišničnem srčnem zastoju in preživetju pacientov, ki so doživeli zunajbolnišnični srčni zastoj ter bili oživljani na teritorialnem območju ene od predbolnišničnih enot NMP v Sloveniji, v letih 2018 in 2019. Za statistično obdelavo podatkov smo uporabili programsko orodje SPSS (IBM SPSS Statistics Version 25), numerične spremenljivke smo predstavili s povprečjem in standardnim odklonom. Za primerjavo dveh atributivnih spremenljivk smo uporabili Pearson Chi Square test, za primerjavo numerične in atributivne spremenljivke pa Mann Whitney test. Rezultati: V letih 2018 in 2019 je bilo obravnavanih 157 srčnih zastojev z oživljanjem. V 106 primerih (67%) so bili izvajani TPO pred prihodom ekipe NMP, v 8 primerih (5%) pa je bil uporabljen tudi AED. Vpričo očividcev je nastalo 77(49%) primerov srčnih zastojev. V tej skupini je bilo 62(80%) srčnih zastojev kardialne etiologije, 15(20%) pa nekardialnih. V 35(45%) primerih je prišlo do povrnitve spontane cirkulacije na terenu, do odpusta pa je preživelo 13(17%) pacientov. Sklep: Ugotovili smo statistično pomembne razlike v preživetju bolnikov s kardialno in nekardialno etiologijo. Dokazali smo tudi povezavo med izhodiščno vrednostjo laktata in acidoze ob sprejemu, na preživetje bolnikov po srčnem zastoju. Obstaja tudi statistično pomembna korelacija med vrednostjo delcev PM10 v ozračju in incidenco srčnega zastoja.

Title:Predictors of survival in patients with out of hospital cardiac arrest
Abstract:Introduction: Survival of patients after out of hospital cardiac arrest is influenced by different factors, such as arrival time, etiology of cardiac arrest, BLS and use of AED, previous illnesses, atmospheric pollution, and pronounced lactic acidosis after spontaneous circulation. Methods: In the theoretical part, we used a descriptive method of work with a review of the literature. In the empirical part, we collected and analyzed data on patient with out of hospital cardiac arrest and survival of patients which were resuscitated in the territorial area of Slovenian EMS prehospital team in the years of 2018 and 2019. For statistical data processing we used SPSS (IBM SPSS Statistics Version 25) software tool. Numerical variables were presented with average and standard deviation. We used the Pearson Chi Square test to compare the two attribute variables, and the Mann Whitney test to compare the numerical and attribute variables. Results: Slovenian EMS prehospital team addressed 157 cardiac arrests with resuscitation, that occured in 2018 and 2019. In 106 cases (67%) BLS was performed before the arrival of the EMS team, AED was used and in 8 cases (5%). 77 (49%) cardiac arrest cases occured in front of witnesses. In this group, there were 62 (80%) cardiac arrest of cardiac etiology, and 15 (20%) of non-cardiac etiology. Spontantues circulation was restored on site in 35 (45%) cases, of which and 13 (17%) patients survived to discharge from hospital. Conclusion: We found statistically significant differences in the survival of patients with cardiac and non-cardiac etiology. We also demonstrated there is an association between baseline lactate and acidosis at the time of admission to the hospital, that has impact on survival of the patients who suffered cardiac arrest. . There is also a statistical correlation between the value of PM10 particles in the atmosphere and the incidence of cardiac arrest.
