Title:Merjenje krvnega tlaka pri pacientih z atrijsko fibrilacijo

Abstract:Atrijska fibrilacija je najpogostejša kardiološka aritmija in prizadene 2–3 % odraslih oseb. Atrijska fibrilacija je supraventrikularna tahiaritmija, za katero je značilna nekoordinirana atrijska aktivacija s posledičnim poslabšanjem atrijske mehanske funkcije. Namen raziskave je bil predstaviti problematiko merjenja krvnega tlaka z avtomatskim oscilometričnim merilnikom ter ugotoviti razlike med merjenjem z avtomatskim oscilometričnim merilnikom in s standardnim avskultacijskim merjenjem. Uporabljena je bila kvantitativna metodologija raziskovanja. Raziskava je potekala v enem izmed urgentnih centrov v Sloveniji. V raziskavi je sodelovalo 77 pacientov. Rezultati so bili analizirani s statističnim programom IBM SPSS Statistics, verzija 25. Ugotovljeno je bilo, da obstajajo statistično značilne razlike v vrednostih krvnega tlaka pri merjenju z oscilometrično in avskultatorno metodo pri pacientih, obolelih za atrijsko fibrilacijo. Razlike se kažejo predvsem pri diastoličnem krvnem tlaku, ki je pri oscilometrični metodi višjih vrednosti kot pri avskultatorni metodi. Rezultati nam kažejo, da atrijska fibrilacija vpliva na meritve krvnega tlaka. Na podlagi pregleda svetovne literature je bilo ugotovljeno, da obstajajo tudi posebni merilniki krvnega tlaka, ki bi bili primerni za ambulante družinske medicine, za merjenje krvnega tlaka doma in na triaži urgentnih služb pri pacientih z atrijsko fibrilacijo.

Title:Measurement of blood pressure at patients with atrium fibrilation
Abstract:The master's thesis presents blood pressure values in patients with atrial fibrillation. Atrial fibrillation is the most common cardiac arrhythmia and affects 2-3% of adults. Atrial fibrillation is a supraventricular tachyarrhythmia characterized by uncoordinated atrial activation with consequent deterioration of atrial mechanical functions. The purpose of the research was to present the issue of measuring blood pressure with automatic oscillometric meters and to determine the difference between measuring with automatic oscillometric meters and standard auscultation measurements. Quantitative research methodology was used. The research took place in one of the emergency centers (UC) in Slovenia. 77 patients participated in the study. The results were analyzed with IBM SPSS Statistics, version 25. We found that there are statistically significant differences in blood pressure values when measured by oscillometric and auscultatory methods in patients with atrial fibrillation. The differences are mainly seen in diastolic blood pressure, which is higher in the oscillometric method than in the auscultatory method. The results show that atrial fibrillation affects blood pressure values. From the world literature, we found that there are also special blood pressure monitors that would be suitable for family medicine clinics, for measuring blood pressure at home, and in emergency department triage in patients with atrial fibrillation.
