Title:Poznavanje smernic za odvzem vzorcev hemokultur med medicinskimi sestrami na urgentnih oddelkih

Abstract:Uvod: Hemokulture so temeljna diagnostična metoda za odkrivanje povzročiteljev sepse, zlasti na urgentnih oddelkih. Kontaminacija vzorcev krvi predstavlja velik problem, saj vodi v nepotrebne zaplete pri pacientih in veča stroške zdravstvene oskrbe. Namen diplomskega dela je proučiti na dokazih utemeljen in strokovno pravilen odvzem krvi za hemokulture ter z raziskavo ugotoviti poznavanje sodobnih smernic pravilne izvedbe tega posega med medicinskimi sestrami v kliničnem okolju. Metode: V teoretičnem delu diplomskega dela smo uporabili deskriptivno metodo, s pregledom domače in tuje literature. V okviru empiričnega dela smo izvedli presečno opazovalno raziskavo, z metodo anketiranja. Raziskava je vključevala 64 medicinskih sester urgentnega centra ene izmed slovenskih bolnišnic. Rezultati: Pri odvzemu krvi za hemokulture 51,6 % anketirancev uporablja sterilne rokavice in 48,4 % anketirancev nesterilne rokavice. V 87,5 % primerih je najpogosteje uporabljen antiseptik za razkuževanje pacientove kože 2 % klorheksidin. V vsako stekleničko priporočeno količino krvi 8–10 ml odvzame 43,8 % anketirancev. Velik delež (40,6 %) anketirancev meni, da nima dovolj znanja s področja odvzemov krvi za hemokulture, 85,9 % anketirancev si želi dodatnega strokovnega izobraževanja s tega področja. Razprava in sklep: Ugotavljamo, da imajo anketirane medicinske sestre dobro znanje s področja odvzemov krvi za hemokulture, a obstajajo določene vrzeli v njihovem znanju. Predlagamo redna izobraževanja s tega področja za zaposlene medicinske sestre, saj strokovno pravilno izveden poseg pozitivno vpliva na učinkovito, kakovostno in varno zdravstveno obravnavo pacienta.

Title:Knowledge of guidelines for hemocultures sampling among nurses in emergency departments
Abstract:Introduction: Blood cultures are a fundamental diagnostic method for sepsis pathogens detection, especially in emergency departments. Contamination of blood samples is a major problem, leading to unnecessary complications for patients and increasing the costs of medical care. The purpose of the diploma thesis is to examine the evidence-based and professionally correct blood sampling for blood cultures and investigate the knowledge of current guidelines for the proper conduct of this procedure among nurses in the clinical environment. Methods: In the theoretical part of the diploma thesis, we used a descriptive method to review domestic and foreign literature. As part of the empirical work, we conducted cross-sectional observational research using the survey method. The survey included 64 nurses from the emergency centre of one of the Slovenian hospitals. Results: 51.6% of respondents use sterile gloves, and 48.4% of respondents use non-sterile gloves when taking blood for blood cultures. The most used antiseptic for the patient's skin disinfection is 2% chlorhexidine in 87.5% of cases. The recommended amount of blood of 8-10 ml in each bottle is taken by 43.8% of respondents. A large proportion (40.6%) of respondents believe that they do not have sufficient knowledge of blood sampling for blood cultures, and 85.9% of respondents would like to have additional professional education in this field. Discussion and conclusion: We find that the nurses surveyed have a good knowledge of blood sampling for blood cultures, but there are some gaps in their knowledge. We suggest regular training in this field for employed nurses since the professionally correctly performed procedure positively impacts the patient's effective, quality, and safe medical treatment.
