Title:Obravnava osebe z anafilaktičnim šokom

Abstract:Uvod: Anafilaksija je huda preobčutljivostna reakcija odziv na določeni alergen v okolju, razvija se lahko zelo hitro in ob neustreznem ukrepanju vodi tudi v smrt. Prizadetost osebe, ki je doživela anafilaktični šok, se lahko kaže na več organskih sistemih; dihalih – težko dihanje, bronhospazem, stridor, krvnem obtoku – hipotenzija, tahikardija, koži – urtikarija, srbečica, oteklina, rdečina, gastrointestinalnem traktu – bruhanje, kolike. Metode: Pri pisanju zaključnega dela smo uporabili deskriptivno metodo dela, s sistematičnim pregledom tuje in domače literature na temo: Ukrepi medicinske sestre ob pojavu anafilaktičnega šoka pri odraslih. Iskali smo v podatkovnih bazah: Pub Med, Science Direct, Medline, Cinahl. Članke smo kritično ocenili, naredili analizo in sintezo. Rezultati: Ugotovili smo, da so najpogostejši vzroki za anafilaktični šok hrana, piki žuželk in zdravila. Anafilaktični šok potrdimo s kliničnim pregledom in odvzemom krvi na encim triptazo, ki se poveča med anafilaktično reakcijo. Ukrepi medicinske sestre ob anafilaksiji so, da pacientu izmeri vitalne znake; saturacijo, krvni tlak, utrip. Nato vzpostavi vensko pot za dajanje zdravil, aplicira adrenalin in druga zdravila po naročilu zdravnika. Zdravila, ki se še uporabljajo ob anafilaksiji, so antihistaminiki, glukokortikoidi, kisik in glukagon. Razprava in sklep: Ob pojavu anafilaktičnega šoka sta pomembna hitro ukrepanje in pristop k obolelemu, saj neustrezno in nepravočasno ukrepanje lahko povzroči tudi smrt

Title:Treatment of a person with anaphylactic shock
Abstract:Introduction: Anaphylaxis is a severe hypersensitivity reaction to a certain allergen in the environment, it can develop very quickly and, if not handled properly, leads to death. The impairment of a person who has gone into an anaphylactic shock may manifest in multiple organ systems: respiratory tract – shortness of breath, bronchospasm, stridor; blood circulation – hypotension, tachycardia; skin – urticaria, itching, swelling, redness; gastrointestinal tract – vomiting, colic. Methods: We used a descriptive work method in writing the final paper and conducted a systematic review of foreign and Slovene literature about nurses' measures in the event of an anaphylactic shock in adults. We searched the following databases: PubMed, Science Direct, Medline, Cinahl. We critically evaluated the articles, and conducted an analysis and a synthesis. Results: The most common causes of an anaphylactic shock have been found to be food, insect bites and medications. Anaphylactic shock is confirmed by performing a clinical examination and blood sampling to determine the enzyme tryptase, which increases during an anaphylactic reaction. In the event of an anaphylaxis, nurses have to measure patient's vital signs; saturation, blood pressure, pulse. They then establish a venous route for giving medications and administer adrenaline and other medications as directed by a physician. Other drugs used in the event of an anaphylaxis are antihistamines, glucocorticoids, oxygen, and glucagon. Discussion and conclusion: In the event of an anaphylactic shock, prompt action and approach to the patient are important, as inadequate and untimely action can also lead to death.
