Abstract:	Uvod: Spremljanje in nadzor pacienta v post-operativnem obdobju v domačem okolju s pomočjo telefonskega klica medicinske sestre olajša pacientu prehod med bolnišničnim ter domačim okoljem. S to intervencijo hitreje ugotovimo morebitne komplikacije po operaciji, pacientom pa lahko tudi svetujemo ob pojavu težav. Namen zaključnega dela je opisati pomen telefonskega spremljanja pacienta v post-operativnem obdobju po odpustu iz bolnišnice. Metode: Za izdelavo zaključnega dela smo izvedli sistematični pregled literature, pri tem pa uporabili deskriptivno metodo dela. Za iskanje literature smo uporabili podatkovne baze: Science Direct, PubMed in Cinahl, kjer smo se osredotočili na strokovne članke ter literaturo, ki so se nanašali na telefonsko spremljanje pacienta po operaciji v domačem okolju. Prikaz uporabljenih člankov smo prikazali v PRISMA diagramu. Rezultati: V podrobno analizo smo vključili 11 člankov. Rezultati so pokazali, da so bili pacienti klica medicinske sestre veseli, saj so se lahko tako pogovorili o njihovem zdravstvenem stanju in težavah, s katerimi so se soočali po odpustu v domače okolje. Prav tako se je telefonsko spremljanje pacienta pokazalo kot učinkovito orodje za zgodnje prepoznavanje zapletov po operaciji, izobraževanje pacienta, preverjanje razumljivosti in interpretacije informacij, ki jih je pacient dobil ob odpustu. Razprava in sklep: Glavni pomen telefonskega spremljanja pacienta v domačem okolju v post-operativnem obdobju je ugotavljanje oziroma spremljanje zdravstvenega stanja pacienta, operativnih ran ter potencialnih težav – komplikacij. Prednost spremljanja je dosegljivost in čas medicinske sestre, da pacientu odgovori na vsa zastavljena vprašanja, ga spodbuja, vodi, svetuje ter izobražuje.

Title:	Telephone folow-up of the patient in the post-operative period after discharge from the hospital
Abstract:	Introduction: Monitoring and supervising the patient in the post-operative period over the phone facilitates the transition between the hospital and home environment. With this approach, we advise patients faster when problems arise and identify complications more quickly. The purpose of the final work is to describe the importance of telephone follow-up of the patient in the post-operative period after discharge from hospital. Methods: For the preparation of the final work, we performed a systematic review of the literature, using a descriptive method of work. To search the literature, we used databases: Science Direct, PubMed and Cinahl, where we focused on professional articles and literature related to telephone monitoring of the patient after surgery in the home environment. The presentation of the used articles is shown in the PRISMA diagram. Results: 11 research papers were included in the detailed analysis. These results showed that the patients were happy to receive a call from nurses, as they were able to discuss their state of health and the problems they faced after being discharged to their home environment. The telephone folow-up has been shown to be effective for early patient education, verification of comprehensibility and interpretation of information received upon discharge and early detection of post-operative complications. Discussion and conclusion: The main importance of telephone follow-up of the patient in the home environment in the post-operative period is the identification or monitoring of the patient's health condition, operative wound and potential problems - complications. The advantage of the follow-up is the availability and time of the nurse to answer all questions, encourage, guide, advise and educate the patient.
