Title:Varna pronacija in mobilizacija pacienta z zunajtelesno membransko oksigenacijo

Abstract:Uvod: Zunajtelesna membranska oksigenacija (ECMO) se uporablja kot podpora pri zdravljenju najtežjih bolnikov v enotah intenzivne terapije. Zaradi pozitivnih učinkov se vse pogosteje izvajata tudi pronacija (PP) in mobilizacija takšnih pacientov. Namen zaključnega dela je predstaviti intervencije, ki zagotavljajo varno PP in mobilizacijo pacienta z zunajtelesno membransko oksigenacijo. Metode: Za izdelavo zaključnega dela smo izvedli pregled literature. Pregled znanstvene literature smo izvedli v podatkovnih bazah CINAHL, Pubmed in SAGE. Vključili smo raziskave, ki so opisovale intervencije za varno izvedbo PP in mobilizacije pacienta z zunajtelesno membransko oksigenacijo. Izvedli smo analizo in sintezo vključenih raziskav. Rezultati: V končni pregled smo vključili devet člankov, ki so ustrezali izbranim kriterijem. Rezultati so pokazali, da je za varno PP in mobilizacijo pacienta na ECMO velikega pomena multidisciplinarno načrtovanje, ki mora biti prilagojeno vsakemu pacientu posebej. Zelo pomembno je tudi sodelovanje med vsemi člani tima, ki pa morajo biti usposobljeni za ravnanje z ECMO ter tudi za izvajanje PP in mobilizacije. Pomembne intervencije za varno izvedbo PP in mobilizacije so tudi ocena stanja pacienta, opazovanje pacienta, vbodnih mest in kanil, uporaba protokolov, monitoring in izključitev kontraindikacij. Razprava in sklep: Z uporabo intervencij, ki zagotavljajo varno PP in mobilizacijo ECMO pacienta, lahko preprečimo življenjsko nevarne zaplete in s tem pridobimo vse pozitivne učinke, ki jih imata pronacija in mobilizacija na bolnika.

Title:Safe pronation and mobilization of patients with extracorporeal membrane oxygenation
Abstract:Introduction: Extracorporeal membrane oxygenation (ECMO) is used to support the treatment of the most difficult patients in intensive care units. Due to the positive effects, pronation (PP) and mobilization of such patients are also increasingly performed. The purpose of the bachelor’s thesis is to present interventions that ensure safe PP and patient mobilization with extracorporeal membrane oxygenation. Methods: To write the bachelor’s thesis, we conducted a literature review. We performed a review of the scientific literature in the CINAHL, Pubmed, and SAGE databases. We included studies that described interventions for the safe implementation of PP and patient mobilization with extracorporeal membrane oxygenation. We performed an analysis and synthesis of the included research. Results: We included nine articles that met the selected criteria in the final review. The results showed that for safe PP and mobilization of the patient on ECMO, multidisciplinary planning is of great importance. The latter must be adapted to each patient individually. Cooperation between all team members is also very important. However, they must be trained to handle ECMO as well as to perform PP and mobilization. Assessment of the patient's condition, observation of the patient, injection sites, and cannulas, use of protocols, monitoring, and exclusion of contraindications are also important interventions for the safe implementation of PP and mobilization. Discussion and conclusion: By using interventions that ensure safe PP and mobilization of the ECMO patient, we can prevent life-threatening complications and, thus, obtain all the positive effects that pronation and mobilization have on the patient.
