Title:Na osebo usmerjeni pristop pri zagotavljanju varnosti zdravil pri polifarmaciji med starejšimi osebami

Abstract:Uvod: Zaradi vse večjega staranja prebivalstva se povečuje obolevnost za več kroničnimi boleznimi, posledično tako pride do jemanja več zdravil hkrati, kar vodi do polifarmacije pri starejših osebah. Polifarmacija pa lahko pri starejših osebah povzroči neželene učinke, stranske učinke, negativne interakcije, zastrupitve z zdravili, pogostejše hospitalizacije, invalidnost in smrtnost. Namen zaključnega dela je na podlagi pregleda literature ugotoviti, kako na osebo usmerjeni pristop zagotovi varno uporabo zdravil pri polifarmaciji med starejšimi osebami. Metode: Pregledali smo znanstveno literaturo, ki se nanaša na temo na osebo usmerjenega pristopa pri zagotavljanju varnosti zdravil pri polifarmaciji med starejšimi osebami, v angleškem jeziku ter raziskave stare do 10 let. Članke smo iskali v podatkovnih bazah Medline, ScienceDirect, Web of Science in PubMed. Rezultati: V analizo zaključnega dela je bilo vključenih šest virov. Ugotovili smo, da na osebo usmerjeni pristop zagotovi varno uporabo zdravil pri multimorbidnih bolnikih s polifarmacijo. Z vključitvijo kliničnega farmacevta v obravnavo bolnikov ter pregledom zdravil lahko zmanjšamo neželene učinke, interakcije med zdravili, vse pogostejše hospitalizacije in nepravilno jemanje zdravil. Razprava in zaključek: Na osebo usmerjen pristop bi zmanjšal negativne vplive polifarmacije. Z vključitvijo zdravnika, medicinske sestre, bolnika in kliničnega farmacevta v obravnavo bolnikov bi lahko zmanjšali število tablet in neželene učinke zdravil.

Title:A person-centred approach to ensure the medication safety in polypharmacy among older people
Abstract:Introduction: Due to the increasing population ageing, the morbidity of several chronic diseases is increasing. As a result, several medications are taken simultaneously, leading to polypharmacy among older people. However, polypharmacy can cause adverse effects, side effects, negative interactions, drug poisoning, more frequent hospitalisations, disability and mortality in older people. Based on the literature review, the thesis aims to determine how a person-oriented approach ensures the safe use of drugs in polypharmacy among older people. Methods: We reviewed the scientific literature on a person-oriented approach to ensuring the safety of medicines in polypharmacy among older people in English and research up to 10 years old. Articles were searched in Medline, ScienceDirect, Web of Science and PubMed databases. Results: Six sources were included in the analysis of the final work. We found that a person-centred approach ensures the safe use of medications in multimorbid patients with polypharmacy. By involving the clinical pharmacist in treating patients and examining medicines, we can reduce side effects, interactions between medicines, more frequent hospitalisations and incorrect taking of medicines. Discussion and conclusion: A person-centred approach would reduce the negative impacts of polypharmacy. The number of pills and adverse drug reactions could be reduced by involving the doctor, nurse, patient and clinical pharmacist in patient management.
