Title:Vpliv odmerka vitamina D na funkcionalno stanje in izražanje izbranih genov pri bolnikih z recidivno remitentno multiplo sklerozo

Abstract:Nastanek multiple skleroze (MS) je povezan z genetskimi in okoljskimi dejavniki, med slednjimi tudi z vitaminom D, vendar še ni razjasnjeno, katere vrednosti v serumu so zadostne, kako ga nadomeščati in ali to vpliva na potek MS. Namen raziskave je bil določiti izhodiščni nivo vitamina ter morebitno povezavo z genotipom, nato pa primerjati učinek nadomeščanja v dveh različnih odmerkih na klinične, laboratorijske in genetske parametre. V randomizirano dvojno slepo raziskavo smo vključili 78 bolnikov z recidivno-remitentno MS, pri katerih smo nadomeščali vitamin D v odmerku 1000 ali 4000 enot dnevno. Pred nadomeščanjem smo ugotovili znižane vrednosti vitamina D, ki so bile tudi odvisne od genetske različice rs12785878, katero smo povezali tudi z višjim porastom vitamina D po nadomeščanju. Višji odmerek je bil varen in učinkovitejši. Vpliva nadomeščanja na izražanje izbranih, za protein kodirajočih geno,v nismo potrdili, je pa prišlo do statistično pomembnega zmanjšanja izražanja vnetne miR-155-5p pri obeh skupinah, kar je nova ugotovitev, ki bi lahko nakazovala na protivnetni učinek nadomeščanja. Z raziskavo smo ugotovili znižane vrednosti vitamina D v serumu pred nadomeščanjem, kar je bilo povezano tudi z genetsko različico rs12785878, dokazali smo, da je višji odmerek učinkovitejši in varen za nadomeščanje in da le-to vpliva na izražanje miR-155-5p.

Title:Effect of vitamin D supplementation on functional parameters and expression of selected genes in patients with relapsing-remitting multiple sclerosis
Abstract:Genetic and environmental factors, including vitamin D, appear to play a role in determining risk of multiple sclerosis (MS). The impact of vitamin D supplementation on MS activity remains unclear and optimal serum levels and supplement doses remain controversial. The aim of the study was to determine baseline serum vitamin levels and any potential connection to genotype. Subsequently, the study aimed to compare effectiveness of two supplementation doses on selected clinical, laboratory and genetic parameters. In a randomized double-blind clinical study, 78 patients with relapsing-remitting MS were enrolled. Vitamin D was administered in daily doses of either 1000 or 4000 units. Insufficient initial serum vitamin D were identified, which additionally dependent on genetic variant rs12785878, also associated with better supplementation response. The higher dose was safe and more effective. No effect on the expression of selected genes was detected, but there was a statistically significant decrease in the inflammatory miR-155-5p expression in both supplementation groups. This novel discovery suggests a potential anti-inflammatory effect of supplementation. The study revealed insufficient baseline vitamin D levels, which were also associated with the genetic variant rs12785878. It demonstrated that a higher dose is more effective, but safe. Vitamin D supplementation also influenced the expression of miR-155-5p.
