Title:Simulacije priprave, delovanja in napak sistemov za dovajanje kisika z visokimi pretoki

Abstract:Uvod: Za aplikacijo kisika lahko uporabljamo sisteme z različnimi pretoki, zato je potrebno ustrezno znanje o kisiku in sistemih, ki jih lahko uporabljamo. Nosna kanila z visokimi pretoki je dokaj nov inovativen sistem, ki se uporablja pri bolnikih, ki dihajo spontano in potrebujejo visoke koncentracije kisika. Metode: S pomočjo simulacije sistema za visoke pretoke smo simulirali pripravo, delovanje sistema in pogoste napake na sistemu. V eksperimentalnem delu smo uporabili sistem za dovajanje kisika z visokimi pretoki WILAmed. Za ogrevanje in vlaženje plinov smo uporabili enoto AIRcon Gen2 in Laerdal Airway Management Trainer za simulacijo dihalnega sistema. Rezultati: Dobljeni rezultati kažejo, da lahko simuliramo pripravo medicinskih plinov, sistema in napak na sistemu. Prav tako nam je uspelo preveriti ustrezne tlake medicinskih plinov v inštalacijskem sistemu in sprožiti alarm. Pri simulaciji napak smo se osredotočili na pogoste opozorilne alarme in slikovno prikazane napake na zaslonu ter jih uspešno simulirali. Razprava in zaključek: Višji pretoki bolje ustrezajo zahtevam pacientov, zmanjšujejo anatomski mrtvi prostor in zagotavljajo rahlo pozitiven tlak v zgornjih dihalnih poteh. Neželeni učinki teh sistemov so epistaksa, nelagodje v nosu in suhost. Potrebnih je več raziskav, ki bi natančno opredelile pozitivne in negativne lastnosti ter jih primerjale z neinvazivno ventilacijo.

Title:Simulations of preparation, operation and failure of high-flow oxygen supply systems
Abstract:Introduction: For the application of oxygen, we can use systems with different flow rates, so it is necessary to have adequate knowledge about oxygen and the systems that can be used. The high-flow nasal cannula is a fairly new innovative system used in spontaneously breathing patients who require high concentrations of oxygen. Methods: With the help of a high-flow system simulation, we simulated the preparation and operation of the system, and common system errors. The WILAmed high-flow oxygen delivery system was used. We used the AIRcon Gen2 unit for heating and humidifying gases, and the Laerdal Airway Management Trainer for simulating the respiratory system. Results: The obtained results show that we can simulate the preparation of medical gases, of the system, and system errors. We also managed to check the appropriate medical gas pressures in the installation system and trigger the alarm. When simulating errors, we focused on the frequent warning alarms and the graphically displayed errors on the screen, and successfully simulated them. Discussion and Conclusion: Higher flows better meet patient requirements, reduce anatomic dead space, and provide slightly positive upper airway pressure. The side effects of these systems include epistaxis, nasal discomfort and dryness. More research is needed to precisely define the positive and negative features, and compare them with non-invasive ventilation.
