Title:Spremljanje dinamike živčnega rastnega faktorja v urinu pri bolnicah s prekomerno aktivnim sečnim mehurjem, zdravljenih z zunajtelesno elektromagnetno stimulacijo in mirabegronom

Abstract:V svoji raziskavi smo preučevali vpliv zdravljenja prekomerno aktivnega sečnega mehurja (PASM) z dodatkom zunajtelesne elektromagnetne stimulacije k ustaljeni terapiji z mirabegronom. V raziskavo smo vključili 56 bolnic s PASM in jih prospektivno, enojno slepo randomizirali v dve skupini. Terapevtska skupina (28 bolnic) je prejemala dnevno 50 mg mirabegron ter dvakrat tedensko 20 minutno elektromagnetno stimulacijo medeničnega in sakralnega predela za obdobje osmih tednov. Kontrolna skupina (28 bolnic) je prav tako prejemala dnevno 50 mg mirabegron in placebo elektromagnetno stimulacijo po enaki shemi kot terapevtska skupina. Zanimalo nas je, ali takšna oblika zdravljenja vpliva na izločanje živčnega rastnega faktorja v urinu ter na subjektivne in objektive kazalnike uspešnosti zdravljenja PASM. V obeh skupinah so se po osmih tednih zdravljenja statistično značilno zmanjšali frekvenca, nokturija, debelina stene sečnega mehurja in število tedenskih epizod urinske inkontinence. Med skupinama ni bilo statistično značilnih razlik v frekvenci in nokturiji ob koncu zdravljenja. Upad tedenskih epizod urinske inkontinence je bil po osmih tednih med skupinama statistično značilen (p = 0,013). Poleg tega je ocena IIQ-7 pokazala bistveno večje zmanjšanje rezultata in ocenjeno izboljšanje simptomov bolnic je bilo pomembno višje v terapevtski skupini. Vpliva načina zdravljenja na izločanje NGF v urinu nismo niti potrdili niti ovrgli. S svojo raziskavo smo dokazali, da dodatek ExMI k mirabegronu pri zdravljenju PASM dodatno zmanjša število tedenskih epizod inkontinence, prav tako vodi tudi do boljšega izboljšanja simptomov.

Title:Monitoring of nerve growth factor dynamics in the urine of patients with overactive bladder treated with extracorporeal electromagnetic stimulation and mirabegron
Abstract:Our study evaluated the effectiveness of adding extracorporeal electromagnetic stimulation to mirabegron therapy in the treatment of overactive bladder (OAB). We included 56 patients with OAB and allocated them into two study groups in a single-blind, randomized manner. The treatment group (28 patients) received a daily dose of 50 mg mirabegron and underwent electromagnetic stimulation of the pelvic and sacral areas twice a week for eight weeks. The control group (28 patients) received the same daily mirabegron dose and a placebo electromagnetic stimulation following the same schedule. The aim was to assess the impact of this treatment approach on the urinary excretion of nerve growth factor, as well as subjective and objective measures of OAB treatment success. Both groups experienced statistically significant reduction in daytime urinary frequency, nocturia, bladder wall thickness and number of weekly incontinence episodes after 8 weeks of treatment. There were no statistically significant differences in end-point daytime urinary frequency and nocturia between groups. The reduction of weekly urinary incontinence episodes at week 8 was statistically significant between the treatment and control group (p=0.013). Moreover, IIQ-7 score showed a significantly greater score reduction and patients’ evaluated improvement of symptoms was higher in the treatment group. The effect of the treatment on urinary NGF excretion remains inconclusive. Our research suggests that supplementing mirabegron therapy with extracorporeal electromagnetic stimulation in OAB treatment can further decrease weekly incontinence episodes and lead to more significant symptom improvement.
