Title:Prospektivna raziskava okužbe s človeškimi papilomavirusi pri moških z anogenitalnimi bradavicami in zdravih prostovoljcih

Abstract:Da bi pridobili globlji vpogled v naravni potek anogenitalnih bradavic (AGB) in dinamiko okužbe s HPV, smo v prospektivni raziskavi spremljali moške paciente z novodiagnosticiranimi AGB in zdrave kontrolne preiskovance. Med vstopom in šestimi kontrolnimi obiski (vsakih 2,6 meseca) smo zbrali tkivne vzorce AGB in 1.232 vzorcev dlak iz anogenitalne regije in obrvi. To predstavlja najtesnejše dolgotrajno spremljanje pacientov z AGB doslej. Pacienti so bili zdravljeni v skladu z uveljavljenimi postopki, večinoma krioterapijo. Ugotovili smo tesno povezavo med prisotnostjo HPV6/11 v dlakah in klinično vidnimi AGB, z visoko stopnjo ujemanja na ravni (pod)linij pri posameznih pacientih. Delež pacientov z vidnimi AGB in HPV6/11-pozitivnimi dlakami se je med spremljanjem zmanjševal s podobnimi trendi. Nobena različica HPV6/11 ni bila povezana z večjo stopnjo ponovitve AGB. Kljub zdravljenju je visok odstotek pacientov z AGB doživel eno ali več ponovitev AGB.Pacienti s HPV6/11-pozitivnimi vzorci dlak ali vidnimi AGB na prejšnjem obisku so imeli znatno večje možnosti za pojav vidnih AGB ob naslednjem obisku. Naši izsledki kažejo na potrebo po nadaljnjih raziskavah za poglobljeno razumevanje naravnega poteka okužbe s HPV6/11, za spodbujanje ozaveščenosti o AGB in podporo širokemu dostopu do cepljenja proti HPV za oba spola in širše starostne skupine.

Title:Prospective study of human papillomavirus infection in men with anogenital warts and healthy controls
Abstract:To better understand natural history of anogenital warts (AGW) and dynamic of HPV infection, newly diagnosed AGW male patients were closely followed in a prospective controlled study. During enrollment and six follow-up visits (every 2.6 months), AGW tissue and 1,232 anogenital and eyebrow hair samples were collected. This represents the closest longitudinal monitoring of AGW patients to date. Patients were treated according to standard-of-care, mostly cryotherapy. HPV6/11 prevalence in AGW tissue, patients’ hair samples and controls’ hair samples was 96.8%, 19.9%, and 0%, respectively. A close association between HPV6/11 presence in hairs and clinically visible AGW was observed, with a high degree of matching on the level of (sub)lineages for individual patients. The proportion of patients with visible AGW and HPV6/11-positive hairs declined during follow-up with similar trends. No particular HPV6/11 variant was linked with increased AGW recurrence. Despite treatment, a high percentage of patients experienced post-initial AGW episodes. Patients having HPV6/11-positive hairs or visible AGW at preceding visit demonstrated substantially higher odds for presenting with AGW in subsequent visit. These findings highlight the need for more research to deepen our understanding of HPV6/11 natural history, promote AGW awareness, and support widespread access to HPV vaccination for both genders and wide age groups.
