Title:Primerjava intravenske in inhalacijske sedacije na oddelku intenzivne medicine

Abstract:Uvod: Sedativi so za nego in zdravljenje pacientov na oddelkih intenzivne medicine bistvenega pomena, saj jim omogočajo lažje prenašanje invazivnih postopkov tekom hospitalizacije. Poznamo dva načina aplikacije sedativov, in sicer intravensko in inhalacijsko. Splošni cilj raziskav je bil primerjava načinov sedacije na oddelkih intenzivne medicine. Metode: Izveden je bil sistematični pregled, analiza in sinteza literature s področja sedacije. Iskanje je potekalo v mednarodnih podatkovnih bazah PubMed, ScienceDirect, Web of Science in CINAHL. Iskanje literature ni bilo časovno omejeno. Rezultati: V sistematični pregled literature smo vključili skupno 10 člankov, od tega 9 randomiziranih kliničnih raziskav in en sistematični pregled randomiziranih kliničnih raziskav. Velikost vzorcev je bila od 44 (n = 44) do 6105 (n = 6105), skupno pa je bilo 7134 (n = 7134) pacientov. Vsi pacienti so bili intubirani in mehansko ventilirani. Članke smo ocenili po SIGN metodi. Najvišjo oceno kakovosti (++) smo dodelili 9 raziskavam, spremenljivo oceno (+) pa eni raziskavi. Nobeni raziskavi nismo dodelili nizke (-) ali nesprejemljive (0) ocene. Razprava in sklep: Na podlagi opravljenega sistematičnega pregleda smo ugotovili, da pri dolgotrajni sedaciji pacientov na oddelkih za intenzivno medicino inhalacijska sedacija kaže boljše učinke kot intravenska. Bistvena prednost uporabe sevoflurana kot sedativa je ta, da se pacienti po njegovi ukinitvi hitreje in bolj koordinirano zbudijo kot pri uporabi propofola.

Title:Comparison of intravenous and volatile sedation in intensive care unit
Abstract:Introduction: Sedatives are essential for the care and treatment of patients in the intensive care unit, as they make it easier for the patient to tolerate invasive procedures during hospitalization. There are two kinds of sedatives, namely intravenous and volatile. The overall aim of the research was to compare sedation methods in intensive care units. Methods: A systematic review, analysis, and synthesis of literature in the field of sedation was performed. The search was conducted in the international databases PubMed, ScienceDirect, Web of Science, and CINAHL. The literature search was not time-limited. Results: We included a total of 10 articles in the systematic literature review, of which 9 were randomized clinical trials and 1 was a systematic review of randomized clinical trials. Sample sizes ranged from a minimum of 44 (n = 44) to a maximum of 6105 (n = 6105), for a total of 7134 (n = 7134) patients. All patients were intubated and mechanically ventilated. Articles were evaluated using the SIGN method. We assigned the highest quality rating (+++) to 9 studies, and a variable rating (+) to one study. We have not assigned a low (+) or unacceptable (0) rating to any survey. Discussion and conclusion: Based on a systematic review, we found that volatile sedation shows better effects than intravenous sedation for long-term sedation of patients in intensive care units. A significant advantage of using sevoflurane as a sedative is that patients wake up faster and more coordinated after its discontinuation, while propofol has longer wake-up times
