Title:Posredovanje slabih novic onkološkim bolnikom

Abstract:Uvod: Na onkološkem področju ima komunikacija ključno vlogo pri vzpostavitvi terapevtskega medsebojnega odnosa. V zaključni nalogi smo želeli raziskati področje posredovanja slabih novic in posledic, ki jih te puščajo na medicinskih sestrah, bolnikih in njihovih svojcih. Metode: Opisna metoda dela je bila uporabljena s poudarkom na pregledu znanstvene literature. Vključene so bile podatkovne baze PubMed, CINAHL ter Web of Science. Za prikaz poteka iskanja je bil uporabljen model PRISME. S piramidno hierarhijo dokazov smo ocenjevali moč dokazov in z JBI orodjem podali kritično oceno člankov. Rezultati: Za končno analizo je bilo izbranih 7 člankov, od tega 1 sistematični pregled in analiza, 2 analitični presečni raziskavi in 4 kvalitativne raziskave. S pomočjo tematske analize smo določili kategorije s prostimi kodami. Glavna kategorija se glasi »Posredovanje slabih novic onkološkim bolnikom«, ki je sestavljena iz »Vloga medicinske sestre pri posredovanju slabih novic v povezavi s spreminjajočimi dejavniki« in »Vloga komunikacijskih modelov pri posredovanju slabih novic«. Rezultati kažejo na korelacijo med izboljšanimi izidi zdravljenja in splošnim zadovoljstvom z uspešno komunikacijo in prilagajanjem komunikacijskih modelov na osebo. Diskusija in zaključek: Medicinska sestra ima pomembno vlogo pri posredovanju slabih novic onkološkemu bolniku, saj ga kontinuirano obravnava, vodi, podpira ter motivira skozi proces holističnega zdravljenja.

Title:Breaking bad news to oncological patients
Abstract:Introduction: Communication is critical in establishing a therapeutic mutual relationship in the oncological field. In the assignment we wanted to investigate the area of communicating bad news and its consequences on nurses, patients and their relatives. Methods: The descriptive work method was used, emphasizing review of scientific literature. PubMed, CINAHL and Web of Science were included. The PRISMA model was used to demonstrate the search process. The pyramid hierarchy assessed the strength of evidence; critical assessment was done by the JBI tool. Results: Seven articles were selected for the final analysis. One was a systematic review and analysis, two were analytical cross-sectional studies, and four were qualitative research. With thematical analysis, we determined categories using free codes, the main category being “Communicating bad news to oncology patients”, which consisted of two categories: “The role of nurse in delivering bad news about modifiable factors” and “The role of communication models in delivering bad news”. The results show a correlation between improved treatment outcomes and overall satisfaction with successful communication and adaptation of communication models to each person. Discussion and conclusion: The nurse is essential in delivering bad news to the oncological patient, as she continuously treats, guides, supports and motivates him throughout the holistic treatment.
