Title:Pristop k poškodovani osebi s sumom na zlom medenice na terenu

Abstract:Uvod: Poškodbe v Sloveniji predstavljajo zelo velik problem na področju javnega zdravja in so tretji glavni vzrok za umrljivost. Zlomi medenice so razmeroma redki, predstavljajo le 2–8 % vseh zlomov, vendar se lahko pri bolnikih s hudo politravmo pogostnost poveča na več kot 25 %, zato je pravilno ukrepanje na terenu bistvenega pomena. Metode: Uporabili smo opisno ali deskriptivno metodo dela in dodatno metodo analize ter sinteze. Znanstveno literaturo smo iskali v tujih podatkovnih bazah: PubMed, CINAHL in ScienceDirect. Rezultati: V končno analizo smo vključili 10 raziskav. Ugotovili smo, da sum in diagnostika zlomov medenice predstavljata velik izziv za ekipo nujne medicinske pomoči na terenu. Ocenjevanje resnosti poškodb in obvladovanje izgube krvi ostajata ključnega pomena za te poškodovance, medtem ko je uporaba imobilizacijskega sredstva bistvena za povečanje možnosti preživetja z omejevanjem izgube krvi. Razprava in zaključek: Uporaba imobilizacijskega sredstva ima pomembno vlogo pri stabilizaciji medenice in obvladovanju izgube krvi, ki je pri teh poškodbah bistvenega pomena, ne vpliva pa toliko na končno možnost preživetja poškodovanca. Izrednega pomena pri poškodovancu s hudo travmo ne glede na ugotovitve samega pregleda je razmislek o sami stabilizaciji medenice v zunajbolnišničnem okolju, saj s tem lahko preprečimo in zmanjšamo kakršnokoli tveganje za resno krvavitev medenice.

Title:Approach to an injured person with a suspected pelvic fracture in the prehospital care
Abstract:Introduction: Injuries are a major public health problem in Slovenia and the third leading cause of mortality. Pelvic fractures are relatively rare, accounting for only 2-8% of all fractures, but in patients with severe polytrauma the incidence can increase to more than 25%, so correct intervention in the field is essential. Methods: We used the descriptive or descriptive method of working and the additional method of analysis and synthesis. We searched scientfic literature in foreign databases: PubMed, CINAHL and ScienceDirect. Results: We included 10 studies in the final analysis. We found that suspecting and diagnosing pelvic fractures is a major challenge for the emergency medical team in the field. Assessment of injury severity and management of blood loss remain crucial for these trauma patients. While the use of immobilization is essential to increase the chances of survival by limiting blood loss. Discussion and conclusion: The use of an immobiliser plays an important role in stabilising the pelvis and controlling blood loss, which is essential in these injuries, but does not have a major impact on the ultimate survival of the injured person. In a severely traumatised patient, irrespective of the findings of the examination itself, consideration of pelvic stabilisation in the pre-hospital setting is of paramount importance, as this can prevent and reduce any risk of serious pelvic haemorrhage.
