Title:Kakovost zdravstvene nege in kompetence vodilnih medicinskih sester

Abstract:Uvod: Pandemija COVID-19 je izpostavila ključne izzive v zdravstvenem sektorju, vključno s pomanjkanjem kadrov in potrebo po izboljšanju kompetenc vodilnih medicinskih sester. Te kompetence so ključne za zagotavljanje visoke kakovosti zdravstvene nege, ki temelji na učinkovitem vodenju timov, obvladovanju stresa in preprečevanju izgorelosti zaposlenih. Zaradi vse večjih zahtev po zagotavljanju standardov kakovosti zdravstvene oskrbe so vodstvene veščine postale nepogrešljive za doseganje varne in kakovostne zdravstvene nege. Namen in cilji: Namen raziskave je bil ugotoviti, ali vodilne medicinske sestre razpolagajo z ustreznimi kompetencami za vodenje zaposlenih, ter raziskati, ali so te kompetence povezane s kakovostno zdravstveno nego. Proučili smo, ali obstajajo razlike v zaznavanju kompetenc glede na izobrazbo vodilnih medicinskih sester in ali dodatna izobraževanja ter delovne izkušnje prispevajo k njihovemu razvoju. Metode dela: V raziskavi je sodelovalo 25 vodilnih medicinskih sester iz slovenskih zdravstvenih ustanov. Podatki so bili zbrani s strukturiranim anketnim vprašalnikom, ki je zajemal demografske podatke, samooceno kompetenc, pogostost kazalnikov kakovosti ter povezave med kompetencami in kakovostjo dela. Za preverjanje zanesljivosti vprašalnika je bil uporabljen Cronbachov alfa koeficient, ki je dosegel vrednost 0,893, kar kaže na visoko zanesljivost vprašalnika. Pri analizi podatkov so bili uporabljeni opisna statistika, povprečne vrednosti in standardni odkloni. Uporabljene so bile tudi statistične metode, kot so Spearmanova korelacija, analiza variance (ANOVA) in linearna regresija za določanje povezanosti med spremenljivkami. Rezultati: Rezultati kažejo, da vodilne medicinske sestre na splošno visoko ocenjujejo svoje kompetence, predvsem na področjih načrtovanja, organizacije dela in postavljanja ciljev. Razlike med kompetencami, ki temeljijo na izobrazbi, niso bile statistično značilne (p > 0,05), kar nakazuje, da smer izobrazbe (zdravstvena nega ali management) ni pomembno vplivala na zaznavanje kompetenc. Linearna regresija je pokazala statistično pomembno povezavo med kompetencami vodij in kazalniki kakovosti zdravstvene oskrbe. Razprava: Razprava potrjuje, da so kompetence vodilnih medicinskih sester ključne za kakovostno zdravstveno nego. Vodje z daljšimi delovnimi izkušnjami in dodatnim izobraževanjem so poročale o večji samozavesti pri ocenitvi vodstvenih kompetenc. Kljub temu raziskava ni potrdila razlik glede na smer izobrazbe, kar nakazuje na univerzalnost pridobljenih vodstvenih kompetenc. Omejitve raziskave, kot so majhen vzorec in metoda samoocene, omejujejo generalizacijo rezultatov. Zaključek: Raziskava potrjuje, da kompetence vodilnih medicinskih sester pomembno vplivajo na kakovost zdravstvene oskrbe. Ključne so kompetence na področju organizacije dela, načrtovanja in vodenja timov. Za izboljšanje kakovosti priporočamo razvoj celovitih programov izobraževanja, ki združujejo klinična znanja z vodstvenimi veščinami, ter uvedbo sistematičnega spremljanja kazalnikov kakovosti. Vodilne medicinske sestre igrajo ključno vlogo pri zagotavljanju visokih standardov zdravstvene nege.

Title:The quality of healthcare and competencies of head nurses
Abstract:Introduction: The COVID-19 pandemic has highlighted critical challenges in the healthcare sector, including staff shortages and the need to improve the competencies of head nurses. These competencies are essential for ensuring high-quality healthcare, based on effective team leadership, stress management, and the prevention of employee burnout. With the increasing demands for maintaining quality standards in healthcare services, leadership skills have become indispensable for achieving safe and high-quality patient care. Purpose: The purpose of the study was to determine whether head nurses possess adequate competencies for managing employees and to explore whether these competencies are associated with high-quality healthcare. The research also aimed to examine whether differences exist in the perception of competencies based on the educational background of head nurses and whether additional training and work experience contribute to their development Working methods: The study involved 25 head nurses from Slovenian healthcare institutions. Data were collected using a structured questionnaire, which included demographic data, self-assessment of competencies, frequency of quality indicators, and the relationships between competencies and work quality. To assess the reliability of the questionnaire, Cronbach's alpha coefficient was used, achieving a value of 0.893, indicating high reliability. Data analysis included descriptive statistics, mean values and standard deviations. Statistical methods such as Spearman's correlation, analysis of variance (ANOVA), and linear regression were also used to determine relationships between variables. Results: The results indicate that head nurses generally rated their competencies highly, particularly in the areas of planning, work organization, and goal setting. Differences in competencies based on educational background were not statistically significant (p > 0.05), suggesting that the type of education (nursing or management) did not significantly affect the perception of competencies. Linear regression showed a statistically significant relationship between leadership competencies and healthcare quality indicators, demonstrating that more competent leaders reported better outcomes, such as fewer treatment errors and higher patient and staff satisfaction. Discussion: The discussion confirms that the competencies of head nurses are essential for ensuring high-quality healthcare. Leaders with longer work experience and additional training reported greater confidence in applying leadership competencies. However, the study did not confirm significant differences based on educational background, indicating the universal nature of acquired leadership competencies. The study's limitations, including a small sample size and self-assessment methodology, restrict the generalizability of the findings. Conclusion: The research confirms that the competencies of head nurses significantly influence the quality of healthcare delivery. Key competencies include work organization, planning, and team leadership. To enhance quality, the development of comprehensive training programs that integrate clinical knowledge with leadership skills is recommended, along with the implementation of systematic monitoring of quality indicators. Head nurses play a critical role in ensuring high standards of healthcare.
