Title:Vloga izbranih biokemijskih markerjev in določanja glomerulne filtracije z ioheksolom pri opredelitvi kronične ledvične bolezni pri otrocih

Abstract:Cilj pediatrične nefrologije je dolgoročno ohranjanje ledvičnega zdravja otrok, da bi se slednje nadaljevalo čim dlje v odraslo dobo. Zaradi nenehne rasti otrok in pridobivanja mišične mase je zanesljivo določanje glomerulne filtracije (GF) za oceno napredovanja KLB težavno. Potrebna je dobra metoda merjenja glomerulne filtracije in uporaba zanesljivih enačb za oceno GF. V doktorski nalogi smo želeli v klinično okolje postaviti metodo meritve GF pri otrocih in preizkusiti uporabo novejših bioloških označevalcev za zgodnje ugotavljanje KLB. V raziskavo smo vključili 50 otrok s KLB in 33 zdravih preiskovancev. Pri vseh preiskovancih smo opravili antropometrične meritve in odvzeli kri in urin za določanje novejših bioloških označevalcev. Preiskovancem s KLB smo s pomočjo plazemskega očistka ioheksola izmerili GF in s pomočjo statističnih metod ocenili, če lahko število odvzemov zmanjšamo. Izmerjeno GF smo primerjali z ocenjeno GF na podlagi izračuna z najpogostejšimi enačbami, uporabljenimi v otroški dobi. Ocenili smo uporabnost nekaterih novejših biomarkerjev za oceno napredovanja KLB. Rezultati raziskave so pokazali zanesljivo meritev GF z ioheksolom z dvema ali tremi odvzemi krvi, ob tem pa enačbo CKID2012 kot najbolj ustrezno za oceno GF. Pokazali smo višje vrednosti bioloških označevalcev receptorja faktorja tumorske nekroze 1 in 2 v krvi otrok s KLB in nižje vrednosti uromodulina v urinu pri teh otrocih. Ugotovili smo tudi višje vrednosti mikroRNA miR-126-3p v serumu preiskovancev s KLB.

Title:The role of selected biochemical markers and measurment of glomerular filtration with iohexol in the assesment of chronic kidney disease in children
Abstract:The goal of paediatric nephrology is the long-term preservation of children's kidney health to ensure it extends as far as possible into adulthood. Due to children's continuous growth and increasing muscle mass, especially during puberty, reliably determining glomerular filtration rate (GFR) can be challenging. A robust method for measuring GFR and the use of reliable equations for GFR estimation are essential. In this doctoral thesis, we aimed to implement a GFR measurement method into the clinical setting and evaluate the usefulness of some newer biological markers for early CKD detection. Our study included 50 children with CKD and 33 healthy controls. Anthropometric measurements were performed in all participants, and blood and urine samples were collected to analyse novel biological markers. In CKD patients, GFR was measured using plasma clearance of iohexol, and statistical methods were employed to assess if less samples can be taken. The measured GF was compared with estimated GFR values calculated using the most commonly applied formulas in paediatric practice. We obtained the most reliable equation for estimating GFR, to which we compared newer biomarkers in children with CKD and healthy subjects as well as comparing to GFR. The study results confirmed the reliability of GFR measurement with iohexol clearance using two or three blood samples, identifying the CKID2012 formula as the most suitable for GFR estimation. We observed higher levels of the biological marker tumour necrosis factor receptor 1 and 2 in the blood of children with CKD and lower levels of uromodulin in their urine. Additionally, we found increased serum levels of microRNA miR-126-3p in CKD patients.
