Title:Nealkoholna zamaščenost jeter (NAFLD) in pokazatelji srčno-žilnih bolezni pri hemodializnih bolnikih

Abstract:Uvod: Nealkoholna zamaščenost jeter (NAFLD) in kronična ledvična bolezen sta povezani z večjo srčno-žilno obolevnostjo. Malo je znanega o vlogi NAFLD pri hemodializnih bolnikih, zato je bil namen naše raziskave oceniti povezavo med napredovalo obliko NAFLD, vnetjem, oksidativnim stresom in pokazatelji srčno-žilnih bolezni pri HD bolnikih. Metode dela: Vključili smo 77 hemodializnih bolnikov, ki smo jih glede na ultrazvok trebuha in meritve dvodimenzionalne elastografije jeter strižnih valov (2D-SWE) razdelili na dve skupini: 1) brez NAFLD ali brez napredovale oblike NAFLD (2D-SWE < 9 kPa) in 2) napredovala oblika NAFLD (2D-SWE ≥ 9 kPa). Zbrali smo anamnestične podatke, kri za laboratorijske preiskave, opravili bioimpedančno analizo sestave telesa, izmerili debelino intime medije (IMT) karotidne arterije, določili število aterosklerotičnih plakov, izmerili arterijsko togost in gleženjske indekse. Rezultati: Napredovalo obliko NAFLD smo ugotovili pri 11 bolnikih. Ti bolniki so imeli v primerjavi z drugo skupino statistično značilno višje vrednosti tumorskega nekrotičnega faktorja-α, medcelične adhezijske molekule-1 in 8-hidroksi-2'-deoksigvanozina, nižji fazni kot in avgmentacijski indeks. Vrednosti IMT, števila aterosklerotičnih plakov, hitrosti pulznega vala in gleženjskega indeksa niso bile statistično različne med skupinama. Zaključek: Hemodializni bolniki z napredovalo obliko NAFLD imajo bolj izražene znake oksidativnega stresa, kroničnega vnetja in slabši prehranski status.

Title:Non-alcoholic fatty liver disease (NAFLD) and cardiovascular disease markers in hemodialysis patients
Abstract:Introduction: Non-alcoholic fatty liver disease (NAFLD) and chronic kidney disease are associated with increased cardiovascular morbidity. Little is known about the role of NAFLD in hemodialysis patients. Therefore, the aim of our study was to assess the relationship between advanced NAFLD, inflammation, oxidative stres, and cardiovascular disease markers in HD patients. Methods: We included 77 hemodialysis patients, who were divided into two groups based on abdominal ultrasound and two-dimensional shear wave elastography (2D-SWE) measurement of the liver: 1) no NAFLD or non-advanced NAFLD (2D-SWE< 9 kPa) and 2) advanced NAFLD (2D-SWE≥ 9 kPa). We gathered medical history data, collected blood samples for laboratory tests, performed body composition bioimpedance analysis, measured carotid intima-media thickness (IMT), assessed the number of atherosclerotic plaques, measured arterial stiffness and calculated ankle-brachial index. Results: Advanced NAFLD was confirmed in 11 patients. These patients had significantly higher levels of tumor necrosis factor-α, intercellular adhesion molecule-1, and 8-hydroxy-2'-deoxyguanosine compared to the other group. They also had a lower phase angle and augmentation index. No significant differences were observed between groups in terms of IMT, number of atherosclerotic plaques, pulse wave velocity, or ankle-brachial index. Conclusion: Hemodialysis patients with advanced NAFLD exhibit more pronounced signs of oxidative stress, chronic inflammation, and poorer nutritional status.
