Title:Nekateri biološki označevalci kroničnega vnetja in oksidativnega stresa ter srčno-žilno zdravje otrok s prekomerno telesno težo

Abstract:Namen doktorske naloge je bil oceniti srčno-žilno zdravje pri otroški debelosti z analizo telesne zamaščenosti, kardiometabolnega profila in bioloških označevalcev vnetja in oksidativnega stresa. Analiziranih je bilo 80 otrok (36 deklic), starih od 5 do 18 let, z normalno telesno težo, prekomerno telesno težo ali debelostjo. Iz krvi smo določili biološke označevalce vnetja in oksidativnega stresa ter razširjene preiskave kardiometabolnega profila. Telesna zamaščenost je bila določena s standardno antropometrijo, bioelektrično impedančno analizo in ultrazvočnimi meritvami debeline trebušne maščobe. Izmerjena sta bila tudi krvni tlak in hitrost pulznega vala. Otroci z debelostjo so imeli neugodno telesno sestavo z višjo telesno zamaščenostjo, povišan krvni tlak, zvišane jetrne encime, sečno kislino, slabši lipidni profil ter povišane vrednosti C-reaktivnega proteina, mieloperoksidaze in levkocitov. Presnovni/metabolični sindrom je bil povezan z nižjimi vrednostmi adiponektina. Tako debelost kot presnovni sindrom sta bila povezana z nižjimi vrednostmi vitamina D ter povečanim vnetjem in oksidativnim stresom, vendar vitamin D ni bil posrednik tega procesa. Analiza proteomskega profila je pokazala povečano izražanje gvanilat vezavnega proteina 5 pri levkocitozi, kar nakazuje nov označevalec kroničnega vnetja, povezanega z debelostjo. Pokazatelji telesne zamaščenosti, zlasti trebušna debelost, in mieloperoksidaza so ključni za oceno srčno-žilnega tveganja pri otrocih. Proteomska analiza ponuja nove vpoglede in upravičuje nadaljnje raziskave.

Title:Some biomarkers of chronic inflammation, oxidative stress, and cardiovascular health in children with obesity
Abstract:This doctoral thesis aimed to assess cardiovascular health in childhood obesity through the analysis of adiposity indices, cardiometabolic profile, and biomarkers of inflammation and oxidative stress. The study included 80 children (36 girls) aged 5 to 18 years, with normal weight, overweight, or obesity. Biomarkers of inflammation and oxidative stress, as well as extended cardiometabolic profile tests, were determined from blood samples. Adiposity was assessed using standard anthropometry, bioelectrical impedance analysis, and ultrasound measurements of abdominal fat thickness. Blood pressure and pulse wave velocity were also measured. Children with obesity exhibited higher adiposity indices, elevated blood pressure, increased liver enzymes, uric acid, a worse lipid profile, and higher levels of C-reactive protein, myeloperoxidase, and leukocytes. Metabolic syndrome was associated with lower adiponectin levels. Both obesity and metabolic syndrome were linked to lower vitamin D levels and increased inflammation and oxidative stress, although vitamin D did not mediate this process. Proteomic analysis revealed increased expression of guanylate-binding protein 5 in leukocytosis, suggesting a new marker of chronic inflammation associated with obesity. Adiposity indicators, particularly abdominal obesity, and myeloperoxidase are key to assessing cardiovascular risk in children. Proteomic analysis provides new insights, justifying further research into these markers in childhood obesity and cardiovascular health.
