Title:Pogledi zdravstvenih delavcev na vnaprej načrtovani načrt oskrbe v okviru na osebo osredotočene paliativne oskrbe

Abstract:Uvod: Vnaprej načrtovani načrt oskrbe je temeljni element paliativne oskrbe, saj omogoča vključevanje pacientovih želja in vrednot v načrtovanje prihodnje zdravstvene oskrbe. Pogledi in izkušnje zdravstvenih delavcev pomembno vplivajo na izvajanje vnaprej načrtovanega načrta oskrbe v praksi. Namen zaključnega dela je preučiti percepcijo zdravstvenih delavcev glede vnaprej načrtovanega načrta oskrbe v okviru na osebo osredotočene paliativne oskrbe. Metode: Izveden je bil pregled znanstvene literature v podatkovnih bazah PubMed, CINAHL Ultimate, ScienceDirect, Sage in Web of Science. Uporabljena je bila deskriptivna metoda dela. Uporabljeni sta bili tematska analiza in sinteza. Rezultati: Od skupno 431 zadetkov je bilo v končno analizo vključenih šest kvalitativnih raziskav. Analiza je pokazala, da se zdravstveni delavci pri izvajanju takšnega pristopa soočajo z različnimi izzivi, vključno s pomanjkanjem časa, usposobljenosti in organizacijske podpore. Ključni dejavniki uspešnega izvajanja takšnega pristopa so bili identificirani kot kontinuiran, na osebo osredotočen pristop, učinkovita in empatična komunikacija ter medpoklicno sodelovanje. Razprava in zaključek: Rezultati kažejo, da so za izboljšanje izvajanja potrebni sistematično usposabljanje, jasna razmejitev vlog v zdravstvenem timu in ustrezna institucionalna podpora. Pristop, ki upošteva pacientove vrednote, čustvene odzive in socialni kontekst, lahko bistveno prispeva h kakovosti oskrbe ter k večji avtonomiji pacientov v paliativni oskrbi.

Title:Healthcare professionals´views on a pre-planned care plan in the context of person-centred palliative care
Abstract:Introduction: Ahead-of-time planned care plan is a fundamental element of palliative care, as it allows for the integration of the patient's wishes and values into the planning of future healthcare. The views and experiences of healthcare professionals have a significant impact on the implementation of advance care planning in practice. The purpose of this thesis is to examine the perception of healthcare professionals regarding advance care planning in person-centered palliative care. Methods: A review of the scientific literature was conducted in the databases PubMed, CINAHL Ultimate, ScienceDirect, Sage, and Web of Science. A descriptive method was used. Thematic analysis and synthesis were used. Results: From a total of 431 hits, six qualitative studies were included in the final analysis. The analysis showed that healthcare workers face various challenges in implementing this approach, including a lack of time, training, and organizational support. The key factors for the successful implementation of this approach were identified as a continuous, person-centered approach, effective and empathetic communication, and interprofessional collaboration. Discussion and Conclusion: Results show that systematic training, clear delineation of roles within the health care team, and appropriate institutional support are necessary for improving implementation. An approach that takes into account the patient's values, emotional responses, and social context can significantly contribute to the quality of care and greater autonomy for patients in palliative care.
