Title:Vpliv zmanjšanja soli v prehrani na srčno popuščanje

Abstract:Uvod: Zmanjšanje soli v prehrani je že vrsto let glavni nefarmakološki ukrep za obvladovanje srčnega popuščanja. Čeprav njegova učinkovitost ni dokazana, se to prepričanje še vedno uporablja v smernicah klinične prakse. Namen zaključnega dela je raziskati, kako zmanjšanje soli v prehrani vpliva na pacienta s srčnim popuščanjem. Metode: V zaključnem delu smo s pomočjo pregleda literature ugotavljali vpliv zmanjšanja soli oz. natrija pri srčnem popuščanju. Pregledali, analizirali in sintetizirali smo znanstveno literaturo, ki smo jo iskali v mednarodnih bazah podatkov: PubMed, ScienceDirect in Cochrane Library, s pomočjo iskalnega niza, ki se je glasil: ("salt reduction" OR "salt restriction" OR "reducing salt" OR "low sodium diet") AND (eating OR diet OR feeding) AND ("heart failure" OR "congestive heart failure") AND (adults OR patient). Uporabili smo deskriptivno metodo dela. S pomočjo diagrama PRISMA smo prikazali potek pregleda raziskav. Opravili smo narativno sintezo vsebine. Rezultati: V končno analizo smo vključili 6 raziskav. Ugotovili smo, da v večini primerov zmanjšanje ali omejitev soli ali natrija povzroči pogostejše hospitalizacije in umrljivost kot zmeren vnos. Po drugi strani pa lahko zmanjšan vnos soli ugodno vpliva na simptome srčnega popuščanja, izboljša laboratorijske izvide (NT-proBNP), koristi pri klasifikaciji NYHA in izboljša kakovost življenja. Razprava in zaključek: Pomemben je individualno prilagojen pristop k pacientu s celovito holistično obravnavo, da se lahko določi raven vnosa soli. Zdrav življenjski slog in ozaveščanje o srčnem popuščanju pomembno vplivata na razbremenitev zdravstvenega sistema in zmanjšujeta stroške zdravljenja.

Title:Effect of dietary salt reduction on heart failure
Abstract:Introduction: For many years reducing salt is one of the main non-pharmacological intervention to manage heart failure. Although it´s effectiveness is not proven, this belief is still used in clinical practice guidelines. The purpose of the final thesis is to investigate how dietary salt reduction in the diet affects patients with heart failure. Methods: In the final thesis, we used a literature review to determine the impact of reducing salt or sodium in heart failure. We reviewed, analyzed and synthesized scientific literature, which we searched in international databases: PubMed, ScienceDirect and Cochrane Library using the seaarch string: ("salt reduction" OR "salt restriction" OR "reducing salt" OR "low sodium diet") AND (eating OR diet OR feeding) AND ("heart failure OR "congestive heart failure") AND (adults OR patient). We used a descriptive methods. Using the PRISMA diagram, we showed the course of the research review. We performed a narrative synthesis of the content. Results: Six studies were included in the final analysis. We found that in most cases reduction or restriction salt or sodium causes more frequent hospitalizations and mortality than moderate intake. On the other hand, reduced salt intake can have a beneficial effect on heart failure symptoms, improve laboratory findings (NT-proBNP), benefit NYHA classification and improve quality of life. Discussion and conclusion: An individualised approach to the patient with comprehensive holistic treatment is important to determine the level of salt intake. A healthy lifestyle and raising awareness of heart failure have a significant impact on relieving the burden on the healthcare system and reducing the cost of treatment.
