Title:Zdravljenje nedonošenčka z zlatenico s fototerapijo

Abstract:Uvod: Zlatenica pri nedonošenčkih je pogost zaplet, ki zaradi nezrele presnove bilirubina pomeni povečano tveganje za nevrološke poškodbe. Čeprav je fototerapija uveljavljen standard zdravljenja, so podatki o njeni učinkovitosti in varnosti pri tej populaciji vedno omejeni. Metode: Izveden je bil pregled znanstvene literature v šestih podatkovnih bazah (PubMed, CINAHL, ScienceDirect, Web of Science, SAGE Journals in COBISS), skladno s smernicami PRISMA. Izvedli smo vsebinsko analizo. Podatki so bili obdelani z deskriptivno in tematsko metodo. Rezultati: Od 3841 zadetkov so bile v analizo vključene štiri študije: dve randomizirani klinični, ena opazovalna in ena klinična smernica. Raziskave potrjujejo učinkovitost fototerapije pri zniževanju bilirubina pri nedonošenčkih. LED-naprave z visoko svetlobno jakostjo in širšo osvetljeno površino so bile klinično učinkovitejše kot drugi svetlobni viri. Ciklična fototerapija je ob enaki terapevtski uspešnosti omogočila znatno zmanjšanje svetlobne izpostavljenosti. Neželeni učinki niso bili zaznani. Razprava: Fototerapija je klinično učinkovit in varen ukrep pri obravnavi zlatenice pri nedonošenčkih, če se izvaja z ustrezno izbiro naprav in pod strokovnim nadzorom. Poleg zniževanja bilirubina postaja ključen tudi vpliv terapije na druge vidike neonatalne oskrbe, kot je ohranjanje materinsko-otroškega stika, ki pomembno vpliva na psihofizični razvoj otroka. Kljub pozitivnim izidom analiziranih študij pa metodološke razlike, majhne velikosti vzorcev in odsotnost dolgoročnega spremljanja izpostavljajo potrebo po nadaljnjih raziskavah.

Title:TREATING INFANT JAUNDICE WITH PHOTOTHERAPY
Abstract:Introduction: Jaundice in preterm infants is a common complication, with an increased risk of neurological damage due to immature bilirubin metabolism. Although phototherapy is an established standard of care, data on its efficacy and safety in this population have always been limited. Methods: A review of the scientific literature in six databases (PubMed, CINAHL, ScienceDirect, Web of Science, SAGE Journals and COBISS) was performed according to PRISMA guidelines. We have conducted a content analysis. Data were processed using descriptive and thematic methods. Results: Of the 3841 hits, four studies were included in the analysis: two randomised clinical, one observational and one clinical guideline. Studies confirm the effectiveness of phototherapy in lowering bilirubin in premature infants. LED devices with high light intensity and wider illuminated area were clinically more effective. Cyclic phototherapy, with the same therapeutic efficacy, allowed a significant reduction of light exposure. No adverse effects were detected. Discussion: Phototherapy is a clinically effective and safe intervention for the management of jaundice in preterm infants, provided that it is performed with appropriate device selection and professional supervision. In addition to lowering bilirubin, the impact of therapy on other aspects of neonatal care, such as maintaining maternal-child contact, which has an important impact on the psychophysical development of the child, is becoming crucial. Despite the positive results of the studies analysed, methodological differences, small cause sizes and the absence of long-term follow-up highlight the need for further research.
