Title:Stališča in prepričanja medicinskih sester v enoti intenzivne terapije o oskrbi ob koncu življenja

Abstract:Uvod: Napredek v intenzivni medicini omogoča vzdrževanje življenja tudi v primerih, ko to ni več združljivo z življenjem ter ni več smiselno. Takšno zdravljenje lahko podaljšuje trpljenje in odpira številna etična vprašanja, zato postaja načrtovanje oskrbe ob koncu življenja vse pomembnejše. Medicinske sestre pri tem igrajo ključno vlogo, a so pogosto premalo vključene v proces odločanja. Cilj: Namen raziskave je bil preučiti izkušnje, prepričanja in stališča medicinskih sester v enotah intenzivne terapije do oskrbe ob koncu življenja, zlasti v povezavi z opustitvijo in odtegnitvijo zdravljenja ter njuno etično enakovrednostjo. Metode: Izvedena je bila kvantitativna raziskava z anonimnim spletno anketnim vprašalnikom zaprtega tipa, ki je bil predhodno že uporabljen in pilotno testiran. Podatki so bili analizirani s programom SPSS. V raziskavo je bil vključen kader zdravstvene nege, ki je zaposlen v enotah intenzivne terapije v UKC Ljubljana. Rezultati: Skupno je bilo izpolnjenih 162 anketnih vprašalnikov, kar predstavlja 59 % ciljne populacije. 97 % anketiranih je že sodelovalo pri oskrbi pacienta, pri katerem je bila sprejeta odločitev o opustitvi ali odtegnitvi zdravljenja, vendar je v samem odločanju sodelovalo le 20 %. Večina jih meni, da sta opustitev in odtegnitev zdravljenja etično enakovredni ter da je nujen pogovor s pacientom in/ali družino. Prav tako se strinjajo s pomenom duhovne oskrbe, učinkovitim lajšanjem bolečin in možnostjo obiskov. Diskusija in zaključek: Rezultati raziskave so primerljivi s predhodnimi rezultati iz tujine. Poudarjajo potrebo po večjem vključevanju medicinskih sester v načrtovanje oskrbe ob koncu življenja ter po dodatnem izobraževanju na tem področju.

Title:Attitudes and beliefs of nurses in the intensive therapy units about end-of life care
Abstract:Introduction: Advances in intensive care medicine have made it possible to sustain life even in situations where the patient's condition is no longer compatible with survival. However, such treatment may prolong suffering and raise important ethical questions, highlighting the growing importance of end-of-life care planning. Nurses play a crucial role in this process, yet they are often insufficiently involved in decision-making. Aim: This study aimed to explore the experiences, beliefs, and attitudes of nurses working in intensive care units regarding end-of-life care. It also examined how nurses perceive the concepts of withholding and withdrawing treatment, and whether they consider them ethically equivalent. Methods: A quantitative research approach was employed using an anonymous online questionnaire. A total of 162 nurses from the University medical center Ljubljana participated. Data were analysed using SPSS and presented descriptively. Results: 97% of respondents had previously provided care to patients for whom treatment was withheld or withdrawn, but only 20% had actively participated in the decision-making process. Most nurses viewed withholding and withdrawing treatment as ethically equivalent and emphasized the importance of communication with patients and/or their families. A majority also supported unrestricted visitation, spiritual care in accordance with patients' beliefs, and effective pain management. Discussion and conclusion: The results are consistent with previous studies from abroad. They underscore the need for greater involvement of nurses in end-of-life care planning and the importance of further education in this field.
