Title:Neprekinjeno spremljanje ravni glukoze v krvi in pogostost hipoglikemije pri odraslih s sladkorno boleznijo tipa 1

Abstract:Najpogostejši akutni zaplet sladkorne bolezni tipa 1 je hipoglikemija, ki pomembno vpliva na kakovost življenja in varnost pacientov. Neprekinjeno spremljanje ravni glukoze se kaže kot učinkovito orodje za zmanjševanje hipoglikemij. Namen diplomskega dela je bil preučiti vpliv neprekinjenega spremljanja ravni glukoze na pogostost hipoglikemije pri odraslih s sladkorno boleznijo tipa 1. Izveden je bil pregled literature v podatkovnih bazah CINAHL Ultimate, PubMed in Web of Science, potek iskanja smo uprizorili s PRISMA diagramom. Vključene so bile raziskave v angleškem jeziku. Moč dokazov smo vrednotili glede na hierarhijo dokazov, raziskave smo predstavili v evalvacijski in sintezni tabeli. Identificiranih je bilo 1089 zadetkov, v sintezo je bilo vključenih 11 raziskav: pet randomiziranih kontroliranih, štiri opazovalne, ena presečna in kvalitativna. Rezultati kažejo, da neprekinjeno spremljanje glukoze zmanjšuje čas v hipoglikemiji, izboljšuje glikemično urejenost in znižuje HbA1c. V kombinaciji z inzulinsko črpalko dodatno zmanjšuje glikemična nihanja v primerjavi s standardnim merjenjem. Rezultati potrjujejo, da neprekinjeno spremljanje ravni glukoze zmanjšuje pogostost in trajanje hipoglikemij ter izboljšuje stabilnost glikemije. Za prihodnje se priporočajo dolgoročne raziskave na raznolikih populacijah, ki bi ocenjevale tudi psihosocialne in ekonomske vidike uporabe.

Title:Continuous glucose monitoring and frequency of hypoglycaemia in adults with type 1 diabetes
Abstract:The most common acute complication of type 1 diabetes is hypoglycaemia, which significantly affects patients’ quality of life and safety. Continuous glucose monitoring has proven to be an effective tool for reducing hypoglycaemia. The aim of this thesis was to examine the impact of continuous glucose monitoring on the frequency of hypoglycaemia in adults with type 1 diabetes. A literature review was conducted in the databases CINAHL Ultimate, PubMed, and Web of Science, and the search process was illustrated with a PRISMA diagram. Studies published in English were included. The strength of evidence was assessed according to the hierarchy of evidence, and the studies were presented in an evaluation and synthesis table. A total of 1,089 records were identified, of which 11 studies were included in the synthesis: five randomized controlled trials, four observational studies, one cross-sectional study, and one qualitative study. The results consistently demonstrate that continuous glucose monitoring reduces time spent in hypoglycaemia, improves glycemic control, and lowers HbA1c levels. When combined with insulin pump therapy, it further decreases glycemic variability compared to standard blood glucose monitoring. Results confirm that continuous glucose monitoring reduces the frequency and duration of hypoglycaemia and improves glycemic stability. For the future, long-term studies in diverse populations are recommended, which would also assess the psychosocial and economic aspects of its use.
