Title:Napovedna vrednost biooznačevalca 14-3-3 v cerebrospinalni tekočini pri diagnosticiranju creutfeldt-jakobove bolezni

Abstract:Uvod: Creutzfeldt-Jakobova bolezen (CJB) je redka, a smrtonosna nevrodegenerativna bolezen, za katero sta značilna hiter potek in zahtevna diagnoza. Kot podporna diagnostična metoda se pogosto uporabljajo biooznačevalci 14-3-3 v cerebrospinalni tekočini, katerih točnost pa se razlikuje med različnimi populacijami. Namen raziskave je ovrednotiti biooznačevalce 14-3-3 v cerebrospinalni tekočini kot diagnostično metodo CJB v slovenski populaciji. Metode: Analizirali smo podatke 180 slovenskih pacientov, pri katerih je bil med letoma 2003 in 2020 izražen klinični sum na CJB. Pri 155 pacientih je bila opravljena analiza biooznačevalca 14-3-3 z metodo ELISA. Podatke smo povezali z nevropatološkimi izvidi pacientov in izračunali njihove diagnostične vrednosti. Rezultati: Občutljivost testa 14-3-3 je znašala med 90,3 % in 96,8 %, specifičnost pa med 63,7 % in 80,6 %, odvisno od klasifikacije šibko pozitivnih rezultatov. Negativna napovedna vrednost je presegala 97 %. Razprava in zaključek: Rezultati kažejo, da negativen izvid z visoko verjetnostjo izključuje CJB, medtem ko pozitiven rezultat zahteva dodatno potrditev z MRI ali EEG. Uporabna vrednost testa za klinično prakso je visoka, zlasti pri hitri diagnostiki. Pravilna ocena mejnih vrednosti biooznačevalcev in uporaba kombinacije več biooznačevalcev bosta izboljšali diagnostiko Creutzfeldt-Jakobove bolezni.

Title:The predictive value of 14-3-3 biomarker in cerebrospinal fluid in diagnosing creutzfeldt-jakob disease
Abstract:Introduction: The Creutzfeldt-Jakob disease (CJD) is a rare but fatal neurodegenerative disorder, characterised by rapid progression and diagnostic complexity. As a supportive diagnostic tool, 14-3-3 biomarkers in cerebrospinal fluid are frequently employed, although their accuracy varies across populations. The aim of this study was to assess the diagnostic value of 14-3-3 biomarkers in cerebrospinal fluid for diagnosing CJD in the Slovenian population. Methods: Data from 180 patients with clinical suspicion of CJD in Slovenia between 2003 and 2020 were analysed. In 155 patients, 14-3-3 biomarkers were measured using the ELISA method. Results were compared with neuropathological findings, and diagnostic values were calculated. Results: The sensitivity of the 14-3-3 test ranged from 90.3% to 96.8%, while specificity ranged from 63.7% to 80.6%, depending on the classification of the weakly positive results. The negative predictive value exceeded 97%. Discussion and Conclusion: The findings suggest that a negative 14-3-3 result reliably excludes CJD, whereas a positive result requires confirmation by MRI or EEG. The test demonstrates high clinical utility, especially in rapid diagnostic settings. Future research should focus on the interpretation of borderline results and the combined application of multiple biomarkers.
