Title:Problematika povezana z dvojno sekvenčno zunanjo defibrilacijo v in-situ okolju

Abstract:Uvod: Zunajbolnišnični srčni zastoj predstavlja enega glavnih vzrokov smrti, pri čemer je zgodnja defibrilacija ključna za preživetje. Dvojna sekvenčna zunanja defibrilacija je obetavna metoda pri refraktarnih ventrikularnih fibrilacijah, vendar prinaša njena izvedba različne izzive. Raziskava zato proučuje dejavnike, ki vplivajo na uspešnost njene izvedbe v in-situ okolju. Metode: Uporabili smo kvalitativno metodologijo z etnografsko metodo opazovanja. Podatke smo zbirali z odkritim opazovanjem in strukturiranim debriefingom po metodi TALK ter jih analizirali s tematsko analizo. Sodelovalo je 20 ekip zdravstvenih delavcev urgentnega centra. Za obdelavo zbranih podatkov smo v programu IBM SPSS Statistics, verzija 22.0, uporabili opisno statistiko in Fisherjev eksaktni test za nominalne spremenljivke. Rezultati: Analiza 20 in-situ simulacij je pokazala, da so glavni izzivi pri izvedbi dvojne sekvenčne zunanje defibrilacije prostorsko-tehnični, časovni, organizacijski in komunikacijski. Razprava in zaključek: Za optimalno izvedbo dvojne sekvenčne zunanje defibrilacije ni dovolj le tehnična pravilnost, temveč so ključni jasna organizacija, predhodna priprava prostora, strukturirana razporeditev nalog in usposabljanje timov. Raziskava poudarja potrebo po standardiziranih protokolih in strukturiranih simulacijah, ki povečujejo varnost, učinkovitost in usklajenost pri izvedbi dvojne sekvenčne zunanje defibrilacije ter ponuja osnovo za izboljšave v klinični praksi.

Title:Issues related to dual sequence external defibrillation in the in-situ setting
Abstract:Introduction: Out-of-hospital cardiac arrest is one of the leading causes of death, with early defibrillation being crucial for survival. Double sequential external defibrillation is a promising method for refractory ventricular fibrillation, however its implementation presents various challenges. This study therefore examines the factors affecting its successful implementation in an in-situ setting. Methods: We used a qualitative methodology with an ethnographic observation approach. Data were collected through open observation and structured debriefing using the TALK method and analyzed using thematic analysis. Twenty emergency care teams participated in the study. We analyzed the collected data using IBM SPSS Statistics version 22.0, applying descriptive statistics and Fisher’s exact test for nominal variables. Results: Analysis of 20 in-situ simulations revealed that the main challenges in performing double sequential external defibrillation were spatial-technical, temporal, organizational, and communicational. Discussion and conclusion: Optimal performance requires not only technical accuracy but also clear organization, prior room preparation, structured task allocation, and team training. The study highlights the need for standardized protocols and structured simulations that enhance safety, efficiency, and coordination in performing double sequential external defibrillation, and provides a basis for clinical practice improvements.
