Title:Telefonsko spremljanje pacienta v pooperativnem obdobju in ponovna hospitalizacija

Abstract:Pooperativno obdobje je ena najobčutljivejših faz zdravstvene obravnave, saj je po odpustu tveganje za zaplete največje. Bolniki pogosto ostanejo brez neposrednega nadzora, kar lahko vodi v pozno prepoznavanje zapletov, nepotrebne obiske urgence in ponovne hospitalizacije. Namen diplomskega dela je preučiti učinkovitost telefonskega spremljanja v pooperativnem obdobju ter njegov vpliv na zgodnje odkrivanje zapletov in zmanjšanje ponovnih hospitalizacij. Pregled literature smo izvedli v podatkovnih bazah PubMed, CINAHL Ultimate in Web of Science. Uporabili smo vključitvene in izključitvene kriterije ter izvedli vsebinsko analizo izbranih znanstvenih člankov. Potek pregleda je prikazan s PRISMA diagramom. V delo smo vključili 12 raziskav, ki so proučevale vpliv telefonskega spremljanja na ponovne hospitalizacije. Nekatere raziskave niso pokazale statistično pomembnega vpliva na dejansko ponovno hospitalizacijo. Večina pa je poročala o večjem zadovoljstvu pacientov in večjem občutku varnosti po odpustu. Telefonsko ali digitalno spremljanje po odpustu je učinkovita podpora pacientom, saj omogoča hitro prepoznavanje težav, preprečevanje zapletov ter zmanjšanje ambulantnih obiskov. Smiselno ga je vključiti v zdravstveno obravnavo, zlasti pri pacientih z večjim tveganjem za zaplete.

Title:Telephone follow-up in the post-operative period and re-hospitalisation
Abstract:The postoperative period is one of the most sensitive phases of healthcare, as the risk of complications is highest after discharge. Patients often remain without direct supervision, which may lead to delayed recognition of complications, unnecessary emergency visits, and hospital readmissions. The aim of this thesis is toexamine the effectiveness of telephone follow-up in the postoperative period and its impact on the early detection of complications and the reduction of readmissions. A literature review was conducted using the PubMed, CINAHL Ultimate, and Web of Science databases. Inclusion and exclusion criteria were applied, and a content analysis of the selected scientific articles was performed. The review process is presented using a PRISMA diagram. Twelve studies examining the impact of telephone follow-up on hospital readmissions were included. Some studies did not demonstrate a statistically significant effect on actual readmission rates. However, most reported higher patient satisfaction and an increased sense of security after discharge. Telephone or digital follow-up after discharge represents effective support for patients, as it enables rapid identification of problems, prevention of complications, and a reduction in outpatient visits. It should be integrated into healthcare practice, particularly for patients at higher risk of complications.
