Title:Zdravstvena nega pri starejših osebah z delirijem

Abstract:Uvod: Delirij je pogost sindrom pri starejših osebah in velikokrat ostane spregledan, zato je vloga medicinske sestre pri prepoznavanju ključna. Namen zaključnega dela je raziskati vlogo medicinske sestre pri zgodnjem prepoznavanju delirija ter predstaviti ukrepe, ki prispevajo k pravočasni intervenciji in boljšim izidom zdravljenja. Metode: Izveden je bil pregled znanstvene literature v podatkovnih bazah: PubMed, Cinahl Ultimate in Web of Science. Upoštevani so bili članki, ki obravnavajo vlogo medicinske sestre pri deliriju pri starejših osebah. Vključene so bile pregledne raziskave, objavljene v zadnjih desetih letih, z jasno opisano metodologijo. Rezultati: Pregled literature kaže, da medicinske sestre pomembno izboljšajo prepoznavanje delirija z uporabo orodij, kot sta Metoda za oceno zmedenosti in Test štirih A-jev. Nefarmakološki ukrepi, kot so reorientacija, zmanjševanje hrupa, podpora spanja, uporaba senzoričnih pripomočkov ter zgodnja mobilizacija, dokazano zmanjšujejo pojavnost delirija. Ključni so tudi stalno opazovanje, učinkovita komunikacija in vključevanje družine. Razprava in zaključek: Kljub dokazani učinkovitosti ostaja prepoznavanje delirija pogosto nezadostno zaradi pomanjkanja znanja, nejasnih protokolov in organizacijskih omejitev. Izboljšave zahtevajo redno izobraževanje, standardizirane postopke in podporo informacijskih sistemov. Zaključimo lahko, da medicinske sestre s strokovnim pristopom bistveno prispevajo k boljši obravnavi in izidom starejših oseb z delirijem.

Title:Nursing care in older people with delirium
Abstract:Introduction: Delirium is a common syndrome in older adults and is often overlooked, which makes the nurse’s role crucial in its detection. The purpose of this thesis is to explore the nurse’s role in the early recognition of delirium and to present interventions that contribute to timely action and improved treatment outcomes. Methods: A review of scientific literature was conducted using the databases PubMed, MEDLINE, and the Cochrane Library. Articles addressing the role of the nurse in delirium in older adults, screening tools, and non-pharmacological interventions were included. Only peer-reviewed studies published in the past ten years and providing a clear methodological description were considered. Results:. A review of the literature shows that nurses significantly improve the recognition of delirium using tools such as the Confusion Assessment Method and the Four A’s Test. Non-pharmacological interventions, such as reorientation, noise reduction, sleep support, the use of sensory aids, and early mobilization, have been shown to reduce the incidence of delirium. Continuous monitoring, effective communication, and family involvement are also key. Discussion and conclusion: Despite proven effectiveness, delirium is still frequently under-recognized due to limited knowledge, unclear protocols, and organizational constraints. Improvements require regular training, standardized procedures, and information system support. It can be concluded that nurses, through their professional approach, contribute significantly to better care and outcomes for older adults with delirium.
