Title:Novi pristopi pri zdravljenju pacientov s hemofilijo A

Abstract:Uvod: Hemofilija A je dedna motnja strjevanja krvi, za katero je značilno pomanjkanje faktorja VIII. Kaže se s ponavljajočimi se krvavitvami v sklepe in mišice. Standardno zdravljenje temelji na nadomeščanju faktorja VIII, vendar ga omejujejo pogoste intravenske aplikacije, razvoj inhibitorjev in vpliv na kakovost življenja. Namen zaključnega dela je s pregledom literature predstaviti in primerjati najnovejše pristope k zdravljenju pacientov s hemofilijo A. Metode: Izveden je bil sistematičen pregled znanstvene literature v podatkovnih bazah PubMed, CINAHL Ultimate in Web of Science. V analizo je bilo vključenih 11 člankov, razvrščenih po hierarhiji dokazov in prikazanih s PRISMA diagramom. Rezultati: Pregled literature je pokazal razvoj novih terapevtskih pristopov, kot so emicizumab, faktorji s podaljšanim razpolovnim časom, terapije za uravnoteženje hemostaze in genska terapija. Emicizumab omogoča subkutano aplikacijo, zmanjšuje pogostost krvavitev ter potrebo po intravenskih infuzijah. Učinkovit je tudi pri pacientih z inhibitorji. Razprava in zaključek: Novi pristopi pomembno izboljšujejo zdravljenje hemofilije A, vendar ostajajo izzivi, povezani z dolgoročno varnostjo, dostopnostjo in obvladovanjem prebojnih krvavitev. Medicinska sestra ima pomembno vlogo pri edukaciji, spremljanju neželenih učinkov in podpori pri samoterapiji.

Title:New approaches to treating patients with hemophilia A
Abstract:Introduction: Haemophilia A is an inherited bleeding disorder characterised by a deficiency of factor VIII. It manifests as recurrent bleeding into joints and muscles. Standard treatment is based on factor VIII replacement therapy; however, it is limited by the need for frequent intravenous administration, the development of inhibitors, and its impact on quality of life. This thesis aims to present and compare the latest approaches to treating patients with haemophilia A through a literature review. Methods: A systematic review of the scientific literature was conducted using the PubMed, CINAHL Ultimate, and Web of Science databases. Eleven articles were included in the analysis, classified according to the hierarchy of evidence, and presented in a PRISMA diagram. Results: The literature review identified several new therapeutic approaches, including emicizumab, extended half-life factor VIII products, haemostasis-rebalancing therapies, and gene therapy. Emicizumab enables subcutaneous administration, reduces bleeding frequency, and decreases the need for intravenous infusions. It is also effective in patients with inhibitors. Discussion and conclusion: New approaches significantly improve the treatment of haemophilia A; however, challenges remain in long-term safety, accessibility, and the management of breakthrough bleeding. Nurses play an important role in patient education, monitoring adverse effects, and supporting self-management.
