Title:Vloga medicinske sestre pri prepoznavanju in spremljanju neželenih učinkov metotreksata pri pacientih z juvenilnim idiopatskim artritisom

Abstract:Povzetek Uvod: Juvenilni idiopatski artritis je najpogostejša kronična revmatična bolezen v otroštvu, metotreksat pa je pogosto uporabljeno osnovno zdravilo, povezano z gastrointestinalnimi, jetrnimi, hematološkimi in vedenjskimi neželenimi učinki. Namen dela je bil opredeliti vlogo medicinske sestre pri njihovem prepoznavanju in spremljanju. Metode: Izveden je bil deskriptivni pregled znanstvene literature v podatkovnih bazah PubMed, CINAHL Ultimate in Web of Science. Vključeni viri so bili analizirani z analitično, primerjalno in narativno-sintezno metodo ter razvrščeni po hierarhiji dokazov. Rezultati: V končno analizo je bilo vključenih 12 virov. Najpogosteje opisani neželeni učinki metotreksata so bili slabost, bruhanje, bolečine v trebuhu, mukozitis, hepatotoksičnost, hematološka odstopanja in intoleranca na zdravilo. Kot ključna področja zdravstvene nege so se izkazali sistematično klinično ocenjevanje, laboratorijsko spremljanje, uporaba standardiziranih orodij, zdravstvena vzgoja ter podpora adherenci. Razprava in zaključek: Medicinska sestra ima osrednjo vlogo pri zgodnjem zaznavanju neželenih učinkov, pravočasnem poročanju in podpori otroku ter družini. Potrebni so jasnejši protokoli za poenoteno spremljanje varnosti zdravljenja z metotreksatom.

Title:The role of nurses in recognizing and monitoring methotrexate adverse effects in patients with juvenile idiopathic arthritis
Abstract:Abstract Introduction: Juvenile idiopathic arthritis is the most common chronic rheumatic disease in childhood, and methotrexate is a commonly used first-line treatment associated with gastrointestinal, hepatic, haematological, and behavioural adverse effects. This study aimed to define the role of nurses in identifying and monitoring these adverse effects. Methods: A descriptive review of the scientific literature was conducted using the PubMed, CINAHL Ultimate, and Web of Science databases. The included sources were analysed using analytical, comparative, and narrative synthesis methods and classified according to the hierarchy of evidence. Results: Twelve sources were included in the final analysis. The most commonly reported adverse effects of methotrexate were nausea, vomiting, abdominal pain, mucositis, hepatotoxicity, haematological abnormalities, and drug intolerance. Systematic clinical assessment, laboratory monitoring, the use of standardised tools, health education, and adherence support emerged as key areas of nursing care. Discussion and Conclusion: Nurses play a central role in the early detection of adverse effects, timely reporting, and support for the child and family. Clearer protocols are needed to ensure consistent monitoring of the safety of methotrexate treatment.
